Poor baby! That looks uncomfortable.

At first, hearing of the fever and looking at the rash (redness and splotches), I was inclined to think roseola. But honestly, something like that could be anything from a viral infection to a severe heat rash to fifth disease or hives from an allergic reaction to antibiotics. Keep an eye on it, mama.

Side note:
@Yellowbeach always helps me identify my weird rashes; perhaps she may be of some help here. She is AH-MAZING with visual identifications of such. (I have autoimmune diseases and allergies. I have asked her 2x.)

I'd worry that it is an amoxicilin reaction as well. Those can be pretty scary so I'd try to get him looked at. If I recall correctly, I believe @amorini dealt with one and her little guy needed to be hospitalized.

@HappyBaker: glad you were able to get in! I was going to say that I know multiple people whose kids did not react to antibiotics right away, but toward the end of the course. Hope it goes away quickly!

@HappyBaker: Just chiming in although it sounds like you already have an answer. Looks just like the reaction we had to amoxicillin too. Ours showed up on the second dose and about 9 days into the 10 day course.

Yep, what @LBee: said. My son got this on amox-on his third or fourth dose of it. And like @jape14's son, he now has a documented penicillin allergy on his chart. It's pretty common.

My DS also developed a rash on his second round of amoxicillin when he was around 8 months. Our pediatrician diagnosed it as an amoxil rash and had us continue with the course of meds. He is now 3 and has had many more rounds of amoxicillin with no issues.

My understanding is that a true allergy is more rare. Might be worth exploring.

@HappyBaker: I don't remember it getting worse, but it definitely took a couple days to settle down. Fingers crossed you figure this out soon!

@HappyBaker: Is there any chance he could also have a fever virus causing a rash? Back in July my DD had a fever, ear infection, and rash and the doctor said it could be from an enterovirus and not the antibiotics. She's had another course of amoxicillin since then without the same issue. Here's what hers looked like - very very similar.

ETA: She needed a round of steroids to clear it up.

@HappyBaker: my son's got a lot worse. we had three pediatrician visits and ended up needing steroids (we tried a combo of benadryl and zyrtec approved by the ped and it didn't touch it). he had rash/hives on the soles of his feet to the point he couldn't walk, his eyes swelled -- it was scary. the steroids SUCKED but it was totally necessary.

@HappyBaker: DD didn't get the rash until her fever broke (thought I'd read in the original post your DS had had a fever?) Benadryl just wouldn't cut it for us either so I ended up bringing her back in and that's when we did the steroids, which acted fast. Poor buddy. Hope he gets some relief soon!

@HappyBaker: Hey! I know I’m late to seeing this but we had this happen when DS was 14 months...I’ll post pictures. Almost identical situation as @Shantuck recalled well. Second round of amox, treating ear infections, and on day 8 of the 10-day dose, the hives started. After seeing us twice in two days for the worsening rash (and changing to Cednifir which may have made it worse due to an effect between the two), our ped had him admitted and we stayed a night in the hospital until things got under control. They diagnosed it as erythema multiforme. They were concerned with it worsening to the point where it was in his mouth, eyes, anus. At that point it can get very serious. In rare cases, it sounds like the skin can become ulcerated and/or impact mucal passages (mouth, sinuses, breathing, pooping).

Fortunately with Zyrtec/Benadryl back and forth at the most frequent doses allowable, under the docs care, it never progressed to that point. We were released from the hospital the following day which happened to be Sunday. DS went back to daycare by mid-week just so that we could monitor things. It did take awhile for all the “hives” to fade.

We saw a drug allergist several months later to followup since DS basically was running out of options for ear infection meds. The allergist said we should wait a year before testing for the allergy again to give him some time to grow out of it. Some kids do, some don’t. It’s really “inconvenient” to not have amox and cednifir as options!

But you don’t need to worry about that now! I would hope things are getting better, but I would definitely escalate this even if your ped doesn’t.
