I have really long hair (butt length) so it's pretty prone to tangles. This is what I do:
1) At night I wear it in a braid, to avoid it getting too tossed around as I sleep
2) When I shower I use a ton of conditioner on the ends of my hair and work out any major tangles with my fingers. I also leave the conditioner in for a few minutes while I wash the rest of myself before rinsing it off.
3) I comb my hair right after my shower while it's still wet. Seems like the easiest way to get tangles out.

I have a repair and detangle spray that I use, by Garnier Fructis. It works great on wet hair and doesn't leave it feeling gross after. My hair gets realllly tangled!

When my hair was really long (and I also had extensions), I would spray in some leave-in conditioner before bed, braid it, and sleep on a silk pillowcase.

I have medium hair (compared to @PUI) and I do very similar to the above except I use a detangling spray. I believe it's made by Aussie, but if you can't find any "adult" kind, you can use the tangle-free spray found in the kid section. I also start my brushing process from the bottom and work my way up.
I have really thick hair and adding layers helped with the tangle problem as well.

I use BioSilk when my hair is wet and never have a problem with tangles. I have long, very thick, wavy hair and don't ever use a brush or comb, just my fingers.

I get them out before I get in the shower. Adding water = disaster. I gently use my fingers, then a natural bristle brush, then a paddle brush. I also use a heavy conditioner and start at the bottom to work through any that formed in the shower.

@MsLipGloss: here too! Tangles come out so much easier when I work from the ends up.

I always brush my hair before I get into the shower. If there's a bad knot I goop it up with conditioner and try to work it out. I comb conditioner through it in the shower and let it sit for 10 minutes. I leave it loose to sleep but spray in a little argan oil to keep it smooth.

I brush my hair before I shower and use a crap ton of conditioner. Then i comb it out before I wrap it in a towel.
I have fine-medium straight hair that wads up if you look at it the wrong way.

I use a ton of conditioner, and also a spray in detangler (it's for kids ). My hair is a little longer than shoulder length, but really fine and tends to tangle and breaks if I'm not gentle.

A good conditioner should help prevent tangles. But "its a 10" is also a miracle product.

I bought a 100% silk pillow case which is supposed to help tangling and frizz. I can't tell if it's effective because I push the pillow out of the way and sleep without one. Time to invest in silk sheets too I guess.
