We saw heartbeats at my 6wk appt, but they acted surprised to see it. They told me not to expect to see much beyond sacs and yolk (I had twins.) I think it really is common to not see a heartbeat at 6wks.

@MrsMed: @MrsMed: Sorry that you're going through such an anxiety-producing time. Apparently late implanters do happen in IVF, though I don't have any personal experience with them. FX you get some really good news on Thursday!!!

@MrsMed: my RE doesn't do an ultrasound until 8 weeks because by then there will be a heartbeat. I asked to come in earlier and they said no, they found it was causing so much stress if there wasn't a heartbeat At 6 weeks, which often times there are not. If your betas doubled, that's a great sign! My beta at 10dp5dt was 88, they like to see it around 100, but then it more than doubled for the next one. I do think they can implant late and that would delay things a day or two. Good luck!

@brady80: thanks! That also makes me wonder how they set the levels at which they like to see the first beta. And if beta levels in early pregnancy matter or is it just doubling. Either way- thanks for the advice!

No ultrasounds here until 7 weeks, so I think you've got some time to see a heartbeat!

So we are doing bloods tomorrow, but I already got in touch with the clinic about what happens next. They were planning another antag cycle (this will be my first time stimming with the new clinic we switched to) but I voiced my concerns, because the last two stims with antag protocols, were freeze alls. We are trying desperately to avoid that again!
Anyway, they've agreed to try back to a long down-reg cycle again. That's what worked for baby Jag back in 2012, so maybe we'll be so lucky again in the future.
Now to just be patient, since this type of cycle takes way longer to get going...

@MrsMed: my RE also will not do a U/S sooner than 7 weeks because of the uncertainty.
In fact, the nurse scheduled me early by accident (I think I was Just at 6 weeks) and my RE was visibly annoyed/mad when he looked at my chart and realized how far along I was. He still did it but it was with a ton of precautionary "we probably won't see anything". And we didn't! Saw a sac and maybe a fetal pole. But he's now my 10.5 month old eating bananas downstairs.

@jaguar: I am glad they listened to your concerns and adjusted accordingly. Hopefully going back to what worked the first time will reap similar success.

@SweetiePie: Thanks for the reassurance. 24 hours to go before the ultrasound. I'm dreading it. These early weeks are new levels of misery.

I am so not caught up on how everyone's doing on this thread, but will definitely pop in more often from here on out!
After weeks (and weeks) of approval and pre-authorization headaches, I start stims for my first IVF cycle tomorrow. Feeling prepared as I can be, and trying to stay positive without raising my hopes too high for the first try.

@agold: Thanks, good luck to you too! When do you think you'll start? I did an estogren priming protocol for a cycle (estradiol starting CD21), but no birth control pills. I'll keep taking it until Sunday. Totally can't remember what it's supposed to do!

I'm doing IUI this month and heading toward IVF if these don't work. Dumb newbie question: do you have to do birth control pills or estrogen as part of IVF? I have severe migraines with estrogen- we joke that I get a migraine if I even look at estrogen.

@AmeliaBedilia: Depends on your protocol. For short, antagonist cycles, you'd skip the pill. For long down-reg cycles, you go on birth control for the first few weeks. As for E2, I only go on those tablets/patches if I'm doing a medicated FET.

@Momazon: Thanks for the shout out! Today did not go well at all. There was no heartbeat. It's so crazy, because I'm fine. Or maybe it hasn't sunk in yet. I mean, I'm upset, but I'm functioning. I think I'd been so preparing for the worst that (I got sick before the dr's visit today.) when he told me, I was glad I didn't have to worry anymore. DH and I are going on a pre-planned romantic getaway this weekend, so I'm looking forward to that. It's our first time away for more than 1 night from our sweet little girl. I have to deal with the D&C next week. Anyone who's been through this have advice about whether to do the pill, a D&C or let it happen naturally? My previous miscarriages we were told to let it happen naturally, so I did. Now I have a choice, I guess.
On a happier note, I want to wish all you ladies starting out your stims @agold and @yogirunner, good luck!

@MrsMed: Oh man, I am just so sorry. I've been there twice and I know what you mean about the worrying being over.
i absolutely recommend a D&C. You are completely out for it so you don't remember a thing and you wake up and it's over. No waiting for the inevitable or worrying you'll be in public or at work when it starts.
My doc gave me the option of pill or D&C and at first I asked for the pill. She was really pushing for D&C and I didn't k ow why. Then I understood. It was hard to take the pill to get it started myself. I was scared and weirdly felt bad, even though there was no life inside of me. So I called her 2 days later and scheduled the D&C. I have no regrets and unfortunately it happened again and I scheduled the D&C without hesitation.

@MrsMed: I'm really sorry to hear that. I don't have any real advice on the D&C questions, as all my losses just happened automatically spontaneously really early on (all started by 6 weeks). Sounds like @SweetiePie has some really solid advice for you, totally makes sense to me. When it does come on "naturally" it can be quite scary - I was at work when it started happening the first time, and it was a huge rush of blood and clots to start and then that continued in the same rushing fashion for several hours during the evening (my husband luckily was there to help get me home). I would definitely make sure you take pads with you on your getaway weekend, and perhaps even wear one as a precaution, just in case things start? If I hadn't have been wearing one when it started (I had one on to deal with the Endometrin leakage loveliness), I completely would have bled through all my clothes.

@AmeliaBedilia: I did three weeks of BCPs each time, leading up to before starting my stims. Estrace pills followed, can't remember exactly when I took them, but I believe I started them around the time of the retrieval or transfer?

@momazon: That's a good point. @MrsMed: even though you schedule a D&C it could still happen on its own before then, so I'd take pads and pain medication just in case. It never happened to me but it did happen to a friend of mine.

@SweetiePie: @momazon: thanks for the advice! I never thought to plan for it happening naturally over the next few days. Now I'll be equipped. They also said they could test the fetal tissue with a D&C. Hopefully we'll learn something. My previous miscarriages came on naturally too and they were relatively early. At least this time we may learn what happened.

@YogiRunner: I'm going to start just as soon as my next cycle starts. I've been having shortish cycles ever since I did IUIs a few months ago. (I had really short cycles when I was taking progesterone... go figure.) I expect my next cycle to start around August 14. I will be out of town from the 13-17th. Yikes. I hope this doesn't interfere with IVF. I'll be so upset if it does. Interesting that you and I are starting off our IVFs differently. I hope the best for both of us! Next question for you: Are you going to be doing a fresh embryo transfer, or freezing all of the embryos?

@AmeliaBedilia: I'm so sorry, but I don't know what has to be done with IVF. I pretty much blindly follow my doctor's instructions until something like an extreme cost snaps me out of my daze.

@MrsMed: Oh yeah! I forgot that is a major benefit to D&C. I had the fetal tissue tested on my 2nd one and that's how they discovered my balanced translocation which caused both miscarriages and will pretty much always cause a miscarriage unless I do IVF w/PGD. That's how I wound up here So had I not done D&C and tested the tissue I wouldn't have my son right now and who knows how many more miscarriages I would have had. Definitely get the testing!

@MrsMed: I am so sorry to hear your update. I would definitely recommend the d&e. We had no heartbeat at 7 weeks and decided to go naturally. I had heavy bleeding and thought it passed. However, 2 weeks later during my follow up, it turned out it never passed and I had to schedule the d&c anyway. I was mad at the time I wasted (among other things). the d&c was "easy" because I was knocked out, and it also gave me a sense of closure. Good luck and take care of yourself.

@AmeliaBedilia: I'm not too sure about e2, but my RE has my on birth control for 7 weeks to 'break the brain/ovary connection.' I HATE bc, I've never been on it before and I'm moody and breaking out like crazy.

@MrsMed: I am so sorry. I had a D+C for a blighted ovum following our first IVF attempt. It wasn't bad...and the meds that they give to try to medically manage a miscarriage apparently don't work for me...at all. I did have genetic testing done on that pregnancy, which was (unfortunately?) "normal." Feel free to wall me if you have questions or just need an extra person to vent.

I had my second consultation today with BWH and he focused more on getting a diagnosis and seemed concerned with the fact that my cycles have always been wildly irregular. He seemed to think that I may have PCOS and wanted to test for testosterone/DHEA and do a ultrasound specifically to look for that though my only symptom is irregular cycles. I thought i twas odd that a baseline CD3 u/s for cycles wouldn't catch anything, but he said that unless they are looking and counting follicles to specifically diagnose it, the baseline CD3 u/s doesnt tell any thing about PCO. Then oddly, when I asked if it was, what would change about a next step and he said nothing, still IVF. So on top of that ,he wanted to do AMH, all the TSH, FSH, E2, etc again. I was going to do it, but then decided I would wait until after my consult with CCRM since I've read they'd make you redo everything anyways. And pushed off my last consult with RMANJ until September.
And 7dpo right now with that wonky gonal cycle.
