I had spotting for one day which I thought was my period starting. I took a pregnancy test because I had a feeling and got a positive. The next day I started miscarrying. :( Only one time we didn't use protection. It was one of the most painful periods I had ever gotten, and I bled through a tampon AND a pad, and was left with a bloodstain on the couch to scrub out as a reminder of the miscarriage.
