I have lean PCOS, and high AMH because I NEVER ovulated (I use PCOS because I've since had a successful IVF pregnancy and have not had AF since birth and am 7 months pp.).

Our course of treatment involved 3 REs, 2 failed IUI (clomid/femara leading to overresponse and cancelled insemination), and then IVF resulting in 1 fresh transfer and 5 frosties awaiting transfer.

I don't share this to scare you, but with our doctor's encouragement we went ahead and did IVF. It made no sense to me to continue trying medicated cycles if I wasn't a prime candidate. From what I've heard, many PCOS women are good candidates for IVF.

@Muggle Born: I see that this post is quite old, but I hope you may still be able to shed some light. I seem to have a similar diagnosis to you: 1) chocolate chip cookie looking ovaries 2) anovulation (no period at all) 3) normal bloodwork and hormone levels.
I have now done 3 rounds of femara which led to ovulation in the first two rounds but no BFP.
I hope you have had one or more sweet babies by now and are enjoying some baby luck. If so, can you please let me know what worked for you? My husband and I have been trying for a year now, and my "lean PCOS" diagnosis only came 5 months ago. I have a hard time finding people with similar symptoms to me, and I am feeling very lost and confused.
Thank you!!

Thank you both.
I have my next appointment with the RE in 2 weeks. I am taking a month off of any medication before I start new rounds of femara. At the appointment, I know we will discuss IUI and the process around that as well as trigger shots next time. Prior to any IUI, my husband is going to go in for SA. I am keeping a list of questions to ask my RE. If you all can think of any good ones, please let me know.
Best of luck to you both! It sounds like progress is being made!

I don't have PCOS but I'll chime in anyways..

I am on my third medicated cycle (1st with Clomid and now on my 2nd round of Femara) and I need the trigger shot to ovulate. Without it my follicles just keep growing until they are over-mature. The trigger shot is super easy and it's nice to have the peace of mind to know when you will ovulate.

Has your husband had a semen analysis? I think that would be a good step before jumping to IUI. I think doing IUI only increases your chances by around 3% per month given there are no male factor issues.

@eg88: Welcome to HB! Sorry that this is what brings you here, but just know that there are many of us who have a very similar experience as you! You've really come to the right place!

I was also diagnosed with PCOS - I'm not overweight, some acne. No BFP with Clomid. With my RE, I did 1 timed intercourse cycle, 3 IUIs and then I got pregnant with our first round of IVF.

It's frustrating when you're on medication and your body still isn't doing what it's supposed to do. For me, though, I needed every. single. step. of my ovulation to be manipulated by medication or else it wouldn't happen. The trigger shot was my friend!

Anyway, I think you'll find lots of success stories on here from people who had a very similar start as you!

Hello Everyone! This is my first post, and I am desperately looking for answers. I am 28 years old and my husband (31) and I have been TTC for a year. After coming off birth control last year, I did not have AF on my own. We also then realized I do not OV on my own. My OB prescribed 50mg Clomid and I ovualted CD18 but no BFP. On my second round of 50mg clomid, no OV. In third month, 100mg did not have successful OV either. I began seeing an RE 6 months ago. He diagnosed me with lean PCOS. My hormone levels, AMH, and bloodwork all came back normal, I am not overweight, no excess or loss of hair, some acne after coming off BCP but has gotten much better, and I have polycystic ovaries. The RE put me on 5mg letrozole in which I ovulated on CD17 two months in a row but no BFP. This past moth was my third month. I have ultrasounds CD13-17 that showed a dominant follicle of 2.6cm on CD17. I also had "OV cramping" on CD17 so we assumed successful OV. My progesterone level test on CD23 revealed I did not ovulate. I am so disappointed and confused. I do not understand how I did not ovualte especially given the dominant follicle. My RE wants me to take a month off and possibly try and HCG trigger shot and even IUI with my next round of letrozole (femara). I have had trouble finding other people with lean PCOS who then have similar symptoms. Any information or advice would be appreciated!
Thank you!

So, my doctor diagnosed me because 1) I do apparently have cysts on my ovaries (from a vaginal ultrasound she said it I have ovaries that look like "choc chip cookies") 2) I'm not ovulating regularly (and probably never have, but I've been on BC for forever, so who knows right?). My labs, interestingly enough, are fairly normal, and she said I just needed annovulatory cycles and cysts on my ovaries to be diagnosed.

I am not going to do a special diet but I am going to cut back on carbs and sugars when I cam, and try to eat more lean proteins and veggies. Nothing extreme. :)

Good luck, ladies!

@Muggle Born: I actually am starting to think i have this as well - I've been off BC since May and my cycles have been sort of out of control - ranging in days from 29 to 47 - but that is really the only symptom I have. I don't have acne, just a few breakouts here and there since coming off BC, no facial hair, but my long cycles lead me to believe there has to be something else going on. Can I ask you how your doctor diagnosed you?

So glad to see this thread. May I ask you guys were officially diagnosed? Did you have an ultrasound to check your ovaries? Blood tests (testosterone, LH/FSH)? I am going to bring it up next annual exam and I want to know what to request.

I have always been lean (5'2 <100lbs), irregular periods, thinning head hair, some errant facial hairs, eat/crave carbs like it's my job etc. To me it adds up but I could just be over thinking things? I do ovulate (I think) probably 3/4 of the time, but the cycles are anything but predictable. We are hoping to really TTC beginning in August and I want to have all of my ducks in a row :)

@Muggle Born: I was diagnosed with "thin" PCOS as well. For me I did change my diet. I did a lot of research since the typical lose 10% of your body fat doesn't really work for someone in the normal BMI range. A lot of PCOS has to do with too much male hormone in the body and eating too many carbs/sugar can lead to a lot of testosterone floating around. So, it can't hurt to cut out refined sugar, simple carbs and wheat. I've also been taking Pregnitude to help with my blood sugar and have ovulated on my own according to my chart.

@Muggle Born: Okay, on the computer now so can write more. I had actually always suspected I had PCOS so came of BC way before we wanted to TTC and low and behold very few periods. Since my DH wasn't ready to TTC I couldn't get medical treatment so I went to an acupuncturist who specialised in fertility. He referred me to a fertility clinic so I could be officially diagnosed and I had a load of tests to confirm. I started having acupuncture once a week and then after about five months added herbs to the mix and my periods came back. I tried cutting out dairy, wheat and sugar too. I couldn't do it completely, but did it as much as I could. Both DH and the acupuncturist urged me to wait a few more months before starting femara and progesterone as the FC had suggested, so I did, then had cycle monitoring on the month we decided to start TTC and that confirmed we were ovulating, and also got us pregnant!!! Gorgeous baby girl is now 12 weeks old and lovely. So, I guess my advice would be to try acupuncture! It was horrible being diagnosed and waiting for the agreed TTC date, I really feel for you at the moment but there is loads of hope. : )

I would try to get more information about your why you received your diagnosis. Did you have your horomone levels tested (FSH/LH, etc.)? Do you have any of the other symptoms of PCOS? I ask because lots of people are misdiagnosed as PCOS -- especially ladies with HA might get a "lean" PCOS diagnosis.

I'm not thin but also have diet-respondent PCOS. In the last couple of months my husband and I have moved to a largely paleo diet, definitely cutting back on the dairy (except fermented, like yoghurt), sugar, wheat and processed grains that @travelgirl1: mentioned. I'm not sure whether it was this, the acupuncture or maca powder, but I've had the clearest luteal surge so far this month. Will keep you posted as to whether it helped me get pg lol.

I recently got diagnosed with "thin" PCOS. I'm 5' 10" and weigh between 135-140 pounds. I have always been thin, but my diet isn't great. My RE didn't really say much when she diagnosed me - just that we would start Clomid on my next cycle for 4-6 months. I'm wondering if I eat healthier or do a specific diet, if it will help. Any lean PCOSers out there? What are you trying, or what worked for you?

I feel very alone in this. :(
