@umlo: there is no clinical test or bloodwork etc that can be done specifically for FPIES. Those thing may be done in order to rule out other possible diagnoses but an FPIES diagnosis is based symptomatically so getting a complete history on your LO was the doctors best and only tool available for diagnosis. If you still want a second opinion I would seek a doctor who specializes in FPIES and see what they had to say/offer.

Thanks all for your comments. I'm sorry for the late reply but wanted to get through our consult with the allergist and she is calling it FPIES, even though she said she thinks its not? I'm very confused and on top of it, the consult was a background on history, diagnosis without an exam/bloodwork/testing and giving me tons of papers and a Rx of miralax for LO constipation.

@anbanan15: did you have the same experience? Is it just the nature of FPIES and the difficulty to diagnose? I am thinking about getting a second opinion.

I met another mom who told me of similar symptoms after rice cereal and her baby was diagnosed with FPIES as well. I hope your LO feels better.

@umlo: No constipation, but yes eczema. I don't think it was related to the rice cereal, though. At 2 he's fine eating all foods, but still has occasional eczema flare-ups more because he seems to have sensitive skin, which is apparently pretty common and for us easily controllable with unscented lotions, soaps, certain sunscreens, oatmeal baths, and hydrocortisone cream.

I hope your daughter feels better soon!

My son has chronic constipation and eczema flares with fails as well. Each kid is SO different though which makes diagnosis even harder.

You could try to eliminate foods from your diet and see if that helps with symptoms. I had to go on a total elimination diet (TED) and was down to 3 foods for awhile. It is so hard, especially to keep your own nutrition up, but for us it was our only option. Once DS self weaned I have been able to eat a normal diet again, but I am struggling with some vitamin deficiencies from how extreme my diet was and from being completely vegan without good protein replacements other than quinoa.

@umlo: Sorry for all the typos. I was posting on my phone but I hope you got what I was saying :) I just wanted to add that the good news is with FPIES your LO should outgrow it at some point!! Literature states 2-3 years, but from parents I've talked to it's more like 4-5 years. Also many kids only have one or two triggers so rice might be your LO's only trigger and reacting via breastmilk like my son did is more rare. Some mom's find they have to eliminate their LO's triggers from their diet, some find they only have to eliminate after their LO started reacting and some find they don't need to at all. With an EBF baby FPIES diagnosis is usually not made until they start solids, around 6 months, because of the low rate of reactions from breastmilk. My son had a ton of suspected food allergy diagnosis's before starting solids with FPIES in the back of our GI's mind. Once he reacted to rice cereal our GI agreed with an FPIES diagnosis. I wrote a guest blog post on HB a few months ago with a little about our experience that you can read here: http://www.hellobee.com/2013/04/05/what-is-your-son-allergic-to/

@umlo: Not to scare you, but I would look into Food Protein Induced Entercolitis Syndrome (FPIES). some good places to start are the International Association for Food Protein Entercolitis (IAFFPE) and the FPIES Foundation websites (I will list links). What you described is CLASSIC FPIES!!! Delayed vomiting post ingestion, usually around the 2 hour mark has FPIES written all over it and rice is one of the most common triggers along with dairy. I would avoid rice cereal at also oat at this point since there is a 50/50 chance your LO would also react to that. Reaction could get worse and the vomiting could lead to shock and lethargy due to dehydration. Has your LO had any blood in his stools or mucous?

I would also stop all solid for a little to allow your LO to have some gut rest before trying any other tin increase the likelyhood of finding a pass. Peas and sweet potatoes are also common trigger foods for FPIES kiddo's.

Just to warn you getting an FPIES diagnosis can be difficult because it is a rather new condition within the medical community and many doctors have not even heard of it before. Others don't regonize it as a proper diagnosis since it is based on symptoms and excluding other possibilities and there are currently no clinical tests for it. You have to fight for your LO on this one! When you talk to your pedi about it come armed with a list of your LO's symptoms, a timeline of what happened, foods he has reacted to, and tons of research papers from the sites mentioned above with the information that pertains to your LO highlighted. What are are you from? Myself or someone else in an FPIES group can help you locate a doctor with FPIES experience in your area.

We have been dealing with our official diagnosis since DS was 6 mo, but looking back we know now je started reacting to foods via my milk at birth. He is now 25 mo. It's been a long road, but we are getting there are we understand better and are under the care of some great allergists and GI specialists within the FPIES community. Feel free to ask me any questions!!!

Here are links to the websites I mentioned and I will also try to link some FPIES groups from FB and BBC for you to check out and ask the mama's on there their opinion about what you described. Collectively these women have years of experience with this disease:

http://iaffpe.org/

http://fpiesfoundation.org/

http://community.babycenter.com/groups/a4859345/fpies_food_protein_induced_enterocolitis_syndrome

https://m.facebook.com/home.php?ref=m_notif&notif_t=feed_comment_reply&__user=24301616#!/home.php?ref=m_notif&notif_t=feed_comment_reply&__user=24301616&soft=side-area

https://m.facebook.com/home.php?ref=m_notif&notif_t=feed_comment_reply&__user=24301616#!/home.php?ref=m_notif&notif_t=feed_comment_reply&__user=24301616&soft=side-area

There are also specific FPIES support groups depending on the area of the country you are in, but the two groups above are the most active.

To answer your first question FPIES is an allergy. It's a food allergy of the gastrointestinal tract that unlike traditional IgE allergies that can cause hives and anaphylaxis, it can cause profuse vomiting with delayed on set, diarrhea, bloody stools, lethargy, and shock among some other more mild symptoms likes sleep disturbances and reflux flares.

@Boogs: that's so great to hear he has been able to tolerate it. When he initially had the vomitting did you also notice any constipation, as if he was super backed up? As well as any eczema? I know each kid is different but just curious since mine has those symptoms as well.

@Rubies: that's comforting to know! I have been researching cutting out dairy in my diet but will try to see if that is truly the culprit.

I was you about a year and a half ago! The same thing happened to LO and his pediatrician said to just skip the rice cereal, continue BFing, and we could just try other solids. After that we never had a problem with LO getting sick like that again and I didn't need to change my diet either.

Rice cereal is how we found out that LO has a milk allergy (skim milk powder is the second ingredient) but she never vomited - she broke out into hives.

I would try just doing one item per meal just to see if you can narrow it down.

Any mamas deal with their LOs projectile vomiting after eating rice cereal? Is this normal? Or a sign of allergy?

We started solids when LO turned 6 months old and she was fussy eating it but ingested it well without any reaction. Fast forward a week later, she ate rice cereal along with peas, sweet potatoes, etc and 2 hours later, BAM. It was so heartbreaking to see her go through the vomit session but once she was done she was back to normal. At that point I just stopped all solids trying to figure it out.

We talked to our Peds and he said some kids do that and the variables are too great to figure out why but encouraged us to put her back on solids again.

We tried again, only giving her less than 1/2 tablespoon and 1 hr later, the vomit sessions began again.

I'm doing my best to figure this out but I am at a loss trying to know where to begin. I know we need to try different foods to see what she can tolerate but I hate knowing that it may possibly make her sick. Rice cereal is a no go anymore but I am wondering if I need to change my diet (I've been EBF but she has had no problem with my milk).
