I would either meet them in person or ask other friends with kids who they would recommend. An application seems super excessive to me!

I just met my potential babysitters in person and asked them all the relevant questions - CPR, background, how long babysitting, comfortable with infants, etc. I also used care . com so I know she had already had a background check and license check.

I recently hired a babysitter and I went through urban sitter, so they are already vetted and have background checks and stuff. And I just had her over to my place for a few minutes. I'm more laid back than most, though.

ETA: I don't know her address or anything because I guess I don't see how that would be relevant. I'm not going to her house...

So we are finally meeting prosepctive babysitters later this week. I have 21 month twins and it has mainly been us and family when we need time off.
I have a new job and I met a co workers friend who gave me some of the numbers to her sitters. Since I don't really know any of these people how do I find out information other than meeting them? Do I have them fill out an application. Thats what I really want to do because I think it is important to know there address, emergency info etc. Or am I crazy?
