I never switched from the slow flow nipples. The estimate is 1oz/hr of separation; however I found that DS needed about 12oz for 10hours away. I was considering the medium flow nipples because those came with the bigger bottles but even the slow flow ones doesn't seem THAT slow. I figured DD may drink more soon and didn't want to buy more bottles later. The TT bottle size between 5oz and 9oz wasn't too different because of the shape so I just went bigger. DD still uses the same bottles now. U should keep slow flow nipples for BM fed babies. The twins are almost 8 months, I leave 1oz per hour of separation - 3oz in am, 4oz at lunch, 3oz in afternoon. This is the case for breastfed babies. That's we did most of the time.
