@AmeliaBedilia: I understand being sick of bad news, I'm so sorry. But I hope that extra monitoring might bring you some more clarity, and help you on the path to being a mum. x

@momazon: That's good to know- I will ask Monday.

My temp is still up at either 12 or 14DPO- BFN. :crying: I am so dreading the next phase- it's like I'm graduating from infertility lite to "serious infertility." I'm so tired of bad news and afraid monitoring will be more bad news.

@AmeliaBedilia: when are they rechecking your prolactin levels? Cabergoline worked really quickly for me - my level went down from 40+ to 5 in less than two weeks of taking it (half a pill twice a week).

IF stinks. I agree that you really need an in-person consult with your RE to talk though the latest results and get clear answers. Immune issues could be the problem. I participate in another forum that's totally anonymous called DC Urban Mom. They have a good Infertility board with lots of information and there have been many immune issues questions. People from out of the area do ask questions and a number of local folks travel to other areas for consultations treatments. Dr. Abassi at Columbia Fertility in DC does some immune work and I've heard many people recommend Dr. Braverman in New York or the Beer Center that I think is in California.

After talking to DH, our plan (assuming I'm not pregnant this cycle) is to go Monday and talk to the RE then. He had talked to me when the last labs came back, and when I asked if we needed to go straight to IVF, he said no but would recommend monitoring next cycle. I don't think this ever so slightly improved AMH changes that. While what I want to do is to give the meds another month to work and just do TI, my only big downsides of IUI and monitoring are worrying about getting bad news (that my AFC will be bad, won't respond well, etc) and "using up" one of the three IUIs before the prolactin and thyroid are back to normal. Cost and time are inconvenient, but not a big deal.

On an upside, my chart looks triphasic- temps jumped at either 7 or 9DPO (o date either day 13 or 15), and has been up for the past four days. I'm really hoping the cabergoline has worked- he said it can work in a couple weeks.

I'm no expert, but I feel like you are right to try to correct the autoimmune piece of this first. After all, it is unlikely that an IUI/IVF cycle would be successful without having addressed this first.

I agree with some of the other ladies that you should book a follow-up with the doctor to discuss your plan and help you decide. Sometimes I find it can be broken telephone when the nurse reiterates the docs message over the phone and they can't answer any of your questions. Wishing you the best of luck, it will all work out soon!!

@Mrs. Pickle: I'm sorry - I did no way mean that with my post! I definitely do not recommend puppies as an IF cure, I just thought it was a funny anecdote. I do truly believe in a mind body connection and recall some powerful studies my sociobiology professor showing an increase in pregnancies in groups of infertile women who attended support groups for IF and those that didn't (I think they were unexplain IF? It was a while ago!). However, I fully realize that IF has many challenges that cannot be overcome on emotional well being alone. Though I think it's worth recognizing that it does play a (even if it's small) part of the big picture. I think that's part of what's so great about HB with all the support and love we have for each other!

@AmeliaBedilia: I'm praying for you, friend, and I'm so sorry you are going through this. I would definitely get a sit down with the RE before making any decisions. Can you meet with him after the ultrasound? I think you'll need to see if/why he's pushing for it to happen NOW. I do know from my personal experience that nurses don't always relay the messages correctly.

@AmeliaBedilia: Ok, just seems as if you are getting conflicting information from the office and not too many people are really well-versed in the issues. I just wanted to help you get success as soon as possible, and I thought you were saying that you were dissatisfied with their lack of guidance on IUI vs IVF and their customer service... thus the suggestion.

@Mrs. Jacks: The nearest place is 6 hours away. My doc seems very well-read on this and laid out a clear, evidence-based medicine plan for this. I also read that prolactin can increase NK cells, and we are working on getting that down.

If you've got high NK cells, you need to be treated by reproductive immunology. I don't know where you are, but there are certain places that specialize in this. Everyone else really doesn't know much. It's a super-specialized new field.

I feel you as I have discovered that my problems are immunologic as well.

@Worrywart: @MenagerieMama: it can be hurtful to hear "relax and you'll get pregnant." Infertility is a medical condition. You can't relax away diabetes any more than you can relax away infertility. I'm glad you were able to get pregnant without intervention, but that isn't always the case.

@AmeliaBedilia: I don't know anything about the issues you are dealing with. I would try to speak to the doctor again and see if he can clarify since you're getting conflicting information. I have to agree with PPs that I would move on to IUI or IVF sooner rather than later especially since you want two LOs.

@AmeliaBedilia: A low AMH is a better predictor of DOR than FSH, as far as I understand. It is more consistent. Considering how long you've been trying, I'd move fairly quickly to IUI or IVF. Especially given that they can take a while too and you want two kids. That's just my take.

Sorry you're going through this, it's so stressful. How are you feeling about the clinic overall? I would definitely question the nurse or ask to speak to someone else.

My vote would be for 2 IUIs (at the most) and then moving to Ivf, especially if you want two children. It's amazing how things can drag out, so I think a more aggressive approach is best. Not to be a total downer, but it may take several ivf cycles. I say this because I was sooo convinced our first ivf cycle would work and it ended up being canceled.

I don't understand all of your numbers, but I would spend a few months trying what you want to make yourself feel comfortable before moving to IVF. Low vitamin D can be fixed with vitamins or a monthly shot (I think). The AMH being that number is something I would want to talk to your doctor about. Like the other girls posted above, the doctor him/herself should be calling you back with the results. Does your doctor recommend IUI with the AMH being that number? You really just need to do what you feel comfortable with. I think every's IF path is so different. If you aren't worried about time/age, could you do a few IUIs before moving on to IVF? I really thought I could get pregnant with an IUI, and I'm glad I tried two of them. But I have no qualms about skipping the third suggested IUI and moving straight to IVF. It was mostly a money and time issue for me. Good luck to you whatever you choose to do!

My RE always followed up with me in person after test results came back - I wouldn't really accept a phone call from a nurse before deciding on a treatment plan. Considering your low AMH in combination with two years of trying, I really wouldn't wait too long before moving onto ART. That said, waiting one or two cycles isn't going to make or break you, if you feel that you'd like to give your meds longer to work or you're just not mentally there yet.

I would start with IUI. I don't know much about your numbers (my issue is PCOS). But I don't think that it can hurt to try. I find your RE's procedures a little weird/unusual though. My doctor usually calls me directly or has me into the office to discuss the plan of action. Nurses can be kind of clueless.

I'm 35 and TTC#1 for two years. We really want two kids. I ovulate like clockwork but 11 day LP.. Labs done 1 year ago were normal, including CD3 FSH 6. Labs didn't include prolactin. Normal HSG. DH's SA 3/2015 showed great numbers but 4% morphology. I was told this was the problem. DH did supplements and avoided his laptop. Recheck 6/2015 showed great numbers and morphology 5%. One negative Femara cycle later, we saw a new RE. He thought the issue was prolactin, started me on cabergoline, and checked labs. My prolactin was 25 (little high), TSH 2.7 (started synthroid), NK cells very high (started prednisone after ovulation), FSH 6 on CD11 during a Femara cycle, and low AMH 0.2. He told me to start melatonin, vitamin D, and DHEA and recheck the AMH. I asked if we should go straight to IVF, and he said no, we should
Monitor next cycle. DH and I were planning on trying a couple cycles with the cabergoline, then IUI until the AMH came back.

The office just called, and said vitamin d was low normal and AMH was 0.27. She clearly didn't know what she was talking about and gave conflicting info. She said he recommended IUI or IVF. I asked about giving the cabergoline time to work, and she said there was no note about that and "you don't have time." I expect AF over the weekend, and scheduled monitoring ultrasound for Monday, a week from today.

I really don't think I have DOR. FSH is great on CD3 and CD11 after Femara. I'm generally healthy and take care of myself. Most of my female relatives have babies into their late 30s.
I really think it's just the prolactin and autoimmune issues, which we just started treating. How urgent is this? Should we do IUI, IVF, or give the meds a little time to work? I'm a mess. Thanks for any advice or positive stories.
