I don't have any first-hand experience with this issues in children, but I have had issues and was diagnosed with idiopathic gastroparesis (delayed gastric emptying). So, I can definitely tell you (from the adult perspective) info about the testing, meds, etc.

I know in adults it can be extremely debilitating. Thankfully, I was able to be successfully treated and I'm doing a lot better.

Thinking of you, but if there's any questions I can answer, let me know!

First-- you are not an asshole. :) we are all just doing the best we can here with the information we have. Also whenever you do barium, don't be concerned about it. Fiona had that with a swallow study and it was nbd!

@twodoghouse: It's probably the "silent" part of it. My lo's was always silent (gurgling, not eating a lot arching) until we tried Pediasure at a year, then full-on huge throwing up. That was a terrible phase. But he definitely did grow out of it! Hindsight is 20/20, so don't sweat it at all. If there's a solution, then great, everyone will be happier.

(And Dylan is still just over 23lbs at 26 months so you're lo's are doing great!)

@Mrs Green Grass: I think we probably got off their reflux meds too early. They stopped spitting up so I thought great! All better! Obviously not, though. I was just desperate for them to be "better." Sigh. I do wonder if their extremely bad sleep all along has been from eating discomfort. I sort of feel like an asshole for making them cry it out now! Size-wise, they are doing well. Blake is 20 lbs, 45% now! Alice actually lost weight between her last appointment and today, but she has been sick for the last month straight (stomach bug and cold). She's at 17 lbs, around 20%.

@Mrs. Tiger: Thanks for the blog links and information about the barium tests. I'm going to read up on this, talk to my husband (he unfortunately couldn't make it to today's appt), and see how we want to proceed. A friend of mine saw another pediatric GI in town so I might see if we can consult with him. Or at least call back the GI I saw today with some more questions. Thanks!

@twodoghouse: I think it's when they give the barium formula and take a bunch of xrays over a few hours to watch it move through their digestive system. I only know about it through this blog I read - don't let her daughter's extreme condition scare you, obviously the fact that your LOs are eating by mouth and having no trouble with formula/bm is great!

My son took Prevacid until about 18 months. He also rarely took more than 2-3 ounces until he was older than one. Solids were also slow for us. He liked chewable foods like puffs, watermelon more than purees. He never really got into them even though we always offered a bit. But eventually everything clicked and he eats normally now.

How are your kids size-wise now?

@Mrs. Tiger: By motility testing, do you mean the upper GI scope and endoscopy? Or are you referring to different tests?

The reason I was given today for waiting on testing is that some babies just take longer for their GI tracts to mature. My kids weren't premature (born at 38+2) but they were small (under 5 and under 6 lbs) for their gestational age. The GI dr wonders if they need more time before they're ready for solids, and since formula and breastmilk is all they really NEED at this point, he thought we should revisit again at 1. He wants us to try solids again in the week or two before our next appointment and see what happens. If we're still having issues, we'll move forward with testing. Since they turn 1 in 6 weeks, I'm generally ok with that. I can't speak to the barium thing - it was just the information he told me today. I don't know why he feels that way, specifically.

However, I'm reading a lot more about DGE and a lot of it sounds familiar. My LOs take many 4 oz bottles throughout the day and often can't finish them. They nurse too, but I doubt I'm providing more than an ounce any time they nurse as my supply is so low. Even when we were doing solids they'd only take a couple spoonfuls of puree or eat a handful of finger foods. Their issues seem tied to quantity, not specific foods, which seems consistent with DGE.

@Mrs Green Grass: Thanks, that's good to know about different meds helping with emptying faster! I may call the dr and ask about that. Blake was on Prevacid previously, but when his reflux seemed "better" we took him off. We just started up the Zantac as an experiment at 9 months and I think my ped chose that one because there's a generic available so it's basically free. I don't know much about the barium, but I'm ok with waiting for the testing since they're only 6 weeks away from 1.

@Mrs Green Grass: I also laughed at the "no barium before 1" comment. D had... four? swallow studies with barium before he was 1 and a few contrasts done before and after his feeding tube was placed. Perhaps he was just in a different boat with different risks1, but its still funny to me. The radiologists commented that the babies love the barium, apparently its sweet?

I'd actually seek a 2nd opinion to see if they should get motility testing done.

We didn't deal with this specifically, but some of the reflux meds I think help empty quicker, so maybe prevacid would work better than Zantac? Each med works differently.

My Lo did a barium swallow study right after birth (maybe since we were still in NICU and trying to get home). No on expressed any concerns about it.

Has anyone's LO dealt with delayed gastric emptying or been diagnosed with Gastroparesis?

My 10.5 month old twins have had ongoing issues with solid foods - mostly major sleep problems after eating solids. Their regular ped suggested that they might be silent refluxing so we put them back on Zantac (they both took reflux meds when they were younger but had outgrown the spit up, back arching, etc.). Zantac has been helping a lot on days that they only have breastmilk and formula (Alice slept through the night last night!) but after solids they wake up 5-7 times (EACH!) and stay up screaming for 45 min-2 hrs some times.

Anyway, because of the sleep issues we are all a little gun shy of going full steam with solids. Today we had a GI appt and he does not believe the solids are causing reflux issues. Apparently it's not very common to have reflux of liquids controlled, but solids cause an issue because solids are literally more solid and harder to reflux. There's no rhyme or reason to what solids are causing the issues (seems to be dependent on qty) so he doesn't think it's allergies. He suggested that we may be dealing with delayed gastric emptying. Testing would require barium which he does not recommend before age one, so we are going to reevaluate after their first birthday. In the meantime, we are going to stick with formula and breastmilk only. I feel really relieved about this because I've been feeling guilty every day that I didn't give them solids even though those days they were much more comfortable and we all slept better! I just hope they eventually can handle solid foods, though!

Has anyone else dealt with this? Did you go through testing? Did your LO eventually outgrow it or did you have to go through some other course of treatment? Thanks!
