@paigeface I really wouldn't lose sleep over it. I'd be ok with it being used if I were pregnant. I'm glad it went well!

Ok! Thanks for the info @toile234! little worried since they used a type c medication. Baby is active and happy in there and was kicking loke crazy while I was there so maybe it all went ok! It wore off really fast too, which was nice.

I'm clearly delayed in responding but in case any future bees look this up ... I had a wisdom tooth pulled during my second trimester. I just had the oral surgeon talk directly with my ob to clear all the drugs being used during the procedure plus pain killers. All went fine.

@paigeface: there's actually no recognized specialty in aesthetic dentistry. Dentists will often bill themselves as cosmetic dentists because they like those procedures, but they did not receive additional schooling for that unless they are prosthodontists which is an actual speciality.

I would actually bring up the epinephrine issue with your ob. Lidocaine, which contains a small amount of epinephrine is a category b drug in pregnancy, and mepivicaine (no epi) is a category c, based on a decrease in fetal heart rate when used. I'm sure you are fine, but in my experience, obs are not the be all and end all for these kinds of things. I've been consistently disappointed in many physicians knowledge of patient management for dental procedures, especially antibiotic prophylaxis. And whenever I try to call a physician to discuss treatment of a shared patient, I often get the run around. I don't expect them to have all of the answers--I don't know very much about kidney diseases, why should they know much about teeth? But I do expect that when I give them clear data on why their current practices for a field that I'm knowledgable in is incorrect, that they read it, and it's not always the case. Ok, rant over!

I didn't have any done because I'm now in my third trimester, but my dentist said if I were in the second trimester, she'd do most dental work. She said the majority is safe, but now that I'm in my third, she just doesn't want to put my on my back or make me uncomfortable for too long, and I'm just to go in after the baby is born. It sounds like the earlier you do it, the better.

Hi ladies! Thanks all so much for the advice and input. My dentist who specializes in aesthetics knew just what to do and NO Epinephrine! Everything went well and I feel better, especially now that the tooth is fixed. The numbing agent wore off really fast without epinephrine, pretty crazy! My OB faxed a letter stating novocaine was fine as long as there was no epinephrine present; mine did not use any! Oh and yes it clearly states NO nitrous oxide while pregnant. Thanks again all!

@lulabee: @mediagirl: my state requires we get recertified in nitrous every two years, and last month when I was taking the online course, the instructor said that there was no evidence that nitrous oxide causes any issues in pregnant women. So in the second trimester for emergent needs it's ok. Does every dentist know this? No. I learned the exact opposite in school, and wouldn't have had any idea it was ok if I had not taken this course and not every state has to. I don't use nitrous in my practice, so it really doesn't hange how I do anything. If the decay is small, I'd just recommend impeccable oral hygiene, fluoride rinse, fluoride toothpaste on your floss, and wait until post partum so that you are more comfortable.
Eta: since I don't use nitrous, I never looked into this any more than just listening to the lecture. Nitrous is still a category c, and they essentially haven't been able to link any issues to nitrous in humans, but they have in rats. My stance is unchanged. If it's not an emergency, just wait until you have the baby and then you can have gas and feel better about the whole situation.

I have to get a filling in my second tri, and I'm nervous about it. Reading this helped. What about laughing gas @toile234: ? I'm a very anxious dental patient anyways with bad jaw issues, I can't get any work done without gas. Can I?

I had a crown replaced with a post during mid-first trimester. We were up against changing insurnace. I had a note from my OB and trusted my dentist (a dad of two). Don't remember what anesthetic was used but no X-rays.

I had two fillings fall out while pregnant with my second. They filled them no big deal after getting the okay from my OB. I had lidocaine, but if my dentist gave me the option I would have went without anything (my old dentist used to give you the option). They were more concerned about me being able to sit in the chair comfortably long enough for the procedure.

@2littlepumpkins: maybe. I'd hope the ob was up on it, but I think they used to teach it differently. I've worked at two different places before I bought my practice, and both of my bosses used mepivicaine for pregnancy. In fact, I got "told on" by an assistant once for using lido, which turned into an awkward education for my boss, though I think he appreciated it. I wouldn't worry if I had mepivicaine while pregnant, but I use lido because it imo works much better and there's no reason not to.
I edited because I was speculating on the reason why they were diff. Categories, but I've been researching but can't find a mechanism as to why, but mepivicaine tends to slow a fetal heart rate. I also can't find when it was first put in that category (the year).

I'm a dentist! Actually, mepivicaine (anesthetic without epinephrine) is a category c drug, and lidocaine (what is usually used as anesthetic) is a category b, so you should actually be using lido for pregnancy, but it seems like a lot of dentists don't know that :-/.
Diagnostic x rays are also fine, just make sure they use an apron/thyroid collar. 2nd trimester is the best time to do non emergent dental work, so don't worry about that.
The real question for me is: did you have the core build up placed and the tooth prepared for a crown and that temporary fell off? Or did the temporary filling after the crown fall out? If it is the latter, the root canal is compromised, and may need redone :-/.
Anyway, a lot of people freak out about dental work during pregnancy, but I don't have any qualms about it and would have just about anything I needed done in the second trimester.

Hi ladies!

I have a tooth that has already had a root canal before I got pregnant but the temporary fell off and the gum has over grown the tooth :( Unfortunatly, it's not pretty and can't hold out until after I give birth. I am nervous to have any work done on my mouth but I have to go in. I know I cannot get anything with epinephrine in it (the stuff that comes along with novocaine) but what are your experiences with dental work in second trimester? Did you have it done? (I'm 20 weeks a 4 days) thank you! :)
