One side has snacks or bottles, the other side has a onesie, a book, and a small toy, and the middle compartment has my cosmetic bag, my wallet, and my cell phone. I purchased a stylish bag so that it would still work as a purse the baby wasn't with me. I definitely use it now that I have two kiddos. I keep extra clothes for both, a swaddle blanket (for nursing cover, drape over car seat, emergency toy for either child, etc) snacks for both and me, a CamelBak that we all share, band aids/ sunscreen/ bug spray, baby toys, and a book for my older one as well as regular purse stuff like wallet and cell phone. It's just easier for me to have everything I need all the time! I use a Petunia PickleBottom Boxy Backpack which is huge but that's what I like. I started with a Skip Hop Duo that I hated from the start (though it makes a great daycare bag now), and then invested in a Kate Spade that I love and still occasionally use if we'll be gone all day. We keep diapers wipes and a change of clothes for each kid in there. DH actually prefers chux pads when we are in public so I keep one or two of those in the bag. We usually change the kids in the back of the minivan so it works for us.
