I went to sleep and felt fine and woke up a few hours later with a 102 fever, the chills, and a red streak on my breast. I think it was partially because I had a serious oversupply at first and we were still in the sleepy newborn phase, and I didn't notice the pain at first because I was still on painkillers from my delivery (broken tailbone = 3 weeks of vicodin). I was using a nipple shield because he was having lunch issues in the shield doesn't let you fully get drained, so I got a blocked duct that turned into mastitis very quickly. I am prone to oversupply, though, so it's not that surprising. In both cases it was when they started lengthening their night sleep (not to anything luxurious, just 4 hours!) I treated with a lot of heat, massage, and pumping when I was really desperate to empty (didn't want to make the oversupply worse). In general it helps to avoid being too engorged for too long...so when my daughter started sleeping through the night at 11 weeks from maybe 7 pm - 7 am, for a while I was waking up in the middle of the night to pump (due to pain but it probably helped prevent issues) and then eventually started pumping last thing before bed (when I'd still get about 4-5 ounces). If I nursed my daughter in the morning and she only took one breast so the other still felt very full/uncomfortable, I also pumped that one. Note: I definitely had some oversupply at that point and the pumping only encouraged the oversupply, but I was ok with some slight oversupply as I wanted to build up a freezer stash for when I went back to work and occasionally had business travel.
