We were not asked on United. I think your chances of needing it are less if she has a seat. If she's flying as a lap child, I'd definitely find and being a copy. Honestly, I'd try and find one anyway. Even if the airlines don't need it, I'd want proof she was my kid if we ended up in a hospital or something while traveling. You can get the short form printed right away in person at your local birth certificate center (not exactly sure what it's called). Ours is located in the same building as the county tax office. The short form will suffice for domestic travel and school enrollment.
