My OB gives you the paperwork at your first pre-natal visit and I filled it out shortly after that. It was strange to fill out paperwork knowing that the information might slightly change by the time it was needed.</p> <p>With DD, the hospital brought me the paperwork I had started (such as the birth certificate form) after I delivered her so that I could finish it.
