@Mrs. Polish: my RE was fabulous and is truly my unborn child's honorary grandfather! I went to school with one of his sons from late elementary to high school. He knew I was scared of IVF with all of the injections and gave us the chance to try Clomid/IUI first even though he knew the odds weren't good for us. He also didn't push us into IVF but gave his opinion when I asked for it. He is one of the top IVF doctors in my area.

I would probably start looking for a new RE if I were in your situation. Did your RE explain why he doesn't want you to try oral meds first?

We have disagreed with our RE many times, and he is very impatient. I love the practice, though, so that is why I stay. Yesterday, for instance, the statistics on twins the doctor gave us for a double embryo transfer didn't seem right. So we called another doctor in the practice for more information (which indeed conflicted). Another example--today, at my egg transfer, the doctor on call told us they were putting in two since they were of medium quality as of this morning. We mentioned that we really would like only one. She went back to the embryologists who said that my embryos had developed during the day and were now of the highest quality so she agreed with us. I truly think that you have to be your own advocate with doctors, who also have other incentives that might differ from your own (like cost-cutting or raising their stats). It is very difficult to find a doctor who will give you the time you need to sort everything own. My own RE has quite visibly expressed impatience with us throughout the course of my treatments due to our diligent question-asking, but these are HUGE decisions we are making! So, in short, stand up to him!

@Mrs Green Grass: I always wondered how Kate wound up with 6! I had heard that she had a cancelled IUI but they had sex anyway, against the advice of her RE, but I didn't know what kind of meds she was on.

Oh, but I definitely have had to really work to pull info out of them. Like, why are we doing this protocol, are there other tests, etc. In the end I always trusted their judgement, but I needed to hear the WHY for my piece of mind, and they didn't always volunteer it.

We had kind of the opposite experience and wanted to move along faster then our RE typically recommends. We were unexplained, tired of waiting, and wanted to do whatever gave us the best odds. But I've heard so many others being pressured, that I so value that our clinic was always more conservative and let us drive the train (within reason, of course).

@Mrs Green Grass: You're right, it is harder to control. We went for injects since we were only doing the one IUI cycle but ended up with 7 follicles. The RE was out of his mind going ahead with the IUI.

If I disagreed, I'd probably switch if that was an option. It's such a difficult process that you need to have faith in your doc. But definitely do what you think is right or ask him why he is pushing injectibles.

My doc didn't like to do injectibles unless really necessary since they are expensive and harder to control outcomes for (that's how Kate got her 6).

I'm not entirely sure if this counts as disagreeing, but my REs office has a policy of taking an OPK at 6:45am & calling their office before 7am to report a + and schedule same day blood work. That's fine and dandy, except the cycle I conceived Chloe I got a negative OPK at 6:45am and a positive at 4:30pm--- so I don't necessarily agree with their timing of when you'll surge.

Maybe another doctor, even though they're not the "best" might be a better fit for you? My RE was pretty flexible and usually gave us options; sometimes I had to ask her what she would recommend because there were pluses and minuses to both courses of treatment.

I also find it weird that your doctor doesn't want to start with oral meds. Have you been on them before? If you want to push back, I would highly recommend Femara. I have incredibly stubborn ovaries and Femara worked much better for me than Clomid. Also, this past cycle (that worked!) I did what's called a combination cycle - started with Femara and then did a few days of Gonal-f. It was less expensive and MUCH less monitoring than purely doing injectables.

From my experience, I would do a Femara cycle first and see how you respond, and then when you know what dose, if you need additional cycles, try combination with injectables instead of jumping straight to all injectables. If you want any more info, let me know :).

I disagreed with our RE/clinic during IVF#1 when they saw a large leading follicle that was most likely a cyst. They didn't tell me until several days into stims, and didn't want to cancel the cycle. I had heard that in situations like that the follicle tends to suppress any other growth, and it turned out I was right...they weren't able to get any eggs from that ovary. It kills me that I didn't insist on canceling and we feel like we wasted $10k on the cycle.

Do you ever disagree with what your RE recommends? Do you stand up to them?

Ours is the best in our area, and I feel like he isn't patient enough/is more willing to jump to more expensive options that we just can't do. I felt that he could have tried a few different oral meds before deciding I needed injects.
