I finally figured out the issue (in our case): The part of the strap towards the bottom, which is out of site, was twisting a little bit, causing one side to be tighter than the other. I don't know how many times I tried to fix it, only to have it happen again the next time we put LO in the seat. I never know why one side seems to be pulling tighter than the other. I always check the lap portion of the belt is taut, but still no dice.
