We take it out of the fridge and warm it in your hand or under warm water just until the fat mixes back in, so not just out of the fridge cold, but not really room temp either. We kept all bottles, whether BM or formula warm until closer to 10 months, when I started making them increasingly tepid in preparation for the transition to cold whole cow's milk. If I give them a bottle and they want more, I usually have to give them some right from the fridge. I could wait but usually one or both are screaming their heads off. They do make faces like they are having a brain freeze of sorts.
