We both went. I think you can do an affidavit but when I read it seemed to suggest that you were attesting that the other parent couldn't be there due to extenuating circumstances. The former lawyer in me knew it was really that DH had to go to work that day, so we just waited until the weekend and got it done at a passport acceptance facility, It's actually the main branch of the public library here in Brooklyn. :) Her picture is pretty funny. It took only a few hours. I was able to apply for it on my own, without DH present. I just had to have certified copies of his identifying documents and there was a place for him to sign on the application, which we filled out ahead of time.
