At my first appt last week my doctor said they are always testing for it in the urine samples. So if it shows up they go ahead and mark you as positive for the rest of your pregnancy and give you meds.

@winniebee: 1st tri i got my pap, and technically they can test for anything, like GBS, bacterial vaginosis, etc, but usually they just say "it's for your pap" even if they test for GBS.
mid 3rd tri is the official GBS swab (only for GBS, no pap). At least that's the guidelines as far as I know, and so far matches my experience.

My first only at the end. My second was a different midwife practice, so they did the urine test. They told me if it shows up in your urine, you are highly colonized and will need antibiotics in labor (they didn't swab me in the third...once you're positive, that's it they said). I was negative for my first, positive for my second. No amount of yogurt changed anything for me. I actually only got one round of antibiotics since I was already 9cm dilated at my 40 week appt and they just wheeled me over to l&d.

Last pregnancy, they only tested toward the end. This time (different practice), they told me based on my initial bloodwork/urine tests that I was positive very early. They will do an additional culture later on but my dr will treat for it during labor no matter what now.

@Shantuck: not sure if it's true but i took probiotics & ate yogurt every day for the month before my test (and until delivery) with the hopes of improving my flora. I tested negative.

@Foodnerd81: yes! The clean catch urine. It's interesting that my practice in Boston, ma; Taos nm, and Portland me all test for it but maybe it's not as common to test As I thought? My nurse said that the numbers were really high in my urine.

With my son, my OB's office tested during the first trimester. I was never given antibiotics and I was never re-tested during the third trimester. When I asked about it, I was told that if I had it then, I would for sure have it at the end of pregnancy and there was no need to re-test. I'm at a new OB this time so I'm not sure how it will work this time around.

@winniebee: oh right! Yeah they probably tested many of us with the pee sample and we just don't realize it. I know that first sample had to be sterile, using the special wipes and stuff first, then after that it was pee in a Dixie cup. So probably they did test mine.

I think the idea is if it shows up in urine that means it's a pretty big infection where the swab would detect a much lower amount of bacteria?

It was part of the 8 week pee sample each time.

The concern is any infection can impact the baby, I guess? There is a certain amount of allowable bacteria down there but if the levels are over a certain level it means infection?

I've only ever been checked towards the middle to end of the third tri. I would probably have refused the earlier check since strep down there is apparently normal (according to what two different providers have told me over the course of two pregnancies) and only something they worry about for L&D. But I also tend to refuse any and all relatively unnecessary procedures.
