Virtually anyone qualifies and your monthly premium is salary based. ie we pay $60/month (it was $30 last year) for M and practically everything is covered including dental until she is 18. DH and I get free insurance through my employer.</p> <p>BTW: I didn't emphasize the not medicaid part in a negative way.
