@Amorini: thank you so much for responding! It’s much worse again today and we haven’t missed any Benadryl doses. A friend who is a nurse saw his pic this morning and said it’s definitely erythmya multiforme at this point so exactly like your sons! Our ped has urgent care hours starting at 8 so we will definitely be heading back today!

@HappyBaker: Hey! I know I’m late to seeing this but we had this happen when DS was 14 months...I’ll post pictures. Almost identical situation as @Shantuck recalled well. Second round of amox, treating ear infections, and on day 8 of the 10-day dose, the hives started. After seeing us twice in two days for the worsening rash (and changing to Cednifir which may have made it worse due to an effect between the two), our ped had him admitted and we stayed a night in the hospital until things got under control. They diagnosed it as erythema multiforme. They were concerned with it worsening to the point where it was in his mouth, eyes, anus. At that point it can get very serious. In rare cases, it sounds like the skin can become ulcerated and/or impact mucal passages (mouth, sinuses, breathing, pooping).

Fortunately with Zyrtec/Benadryl back and forth at the most frequent doses allowable, under the docs care, it never progressed to that point. We were released from the hospital the following day which happened to be Sunday. DS went back to daycare by mid-week just so that we could monitor things. It did take awhile for all the “hives” to fade.

We saw a drug allergist several months later to followup since DS basically was running out of options for ear infection meds. The allergist said we should wait a year before testing for the allergy again to give him some time to grow out of it. Some kids do, some don’t. It’s really “inconvenient” to not have amox and cednifir as options!

But you don’t need to worry about that now! I would hope things are getting better, but I would definitely escalate this even if your ped doesn’t.

@HappyBaker: My LO2 has food allergies and had an allergic reaction when she was around that age. I would probably bring your son back in, mainly because he can't verbalize if he's having breathing issues, etc. I would also want to get steroids just in case - especially going into the weekend. You don't have to use them, but I know @jape14's son was super uncomfortable (he was almost 4 when this happened so could tell her) for DAYS even with the steroids. Little buddy probably is miserable!

@HappyBaker: DD didn't get the rash until her fever broke (thought I'd read in the original post your DS had had a fever?) Benadryl just wouldn't cut it for us either so I ended up bringing her back in and that's when we did the steroids, which acted fast. Poor buddy. Hope he gets some relief soon!

@HappyBaker: my son's got a lot worse. we had three pediatrician visits and ended up needing steroids (we tried a combo of benadryl and zyrtec approved by the ped and it didn't touch it). he had rash/hives on the soles of his feet to the point he couldn't walk, his eyes swelled -- it was scary. the steroids SUCKED but it was totally necessary.

@IRunForFun: Interesting! That looks just like his rash - but he doesn't have a fever and his ears are clear now! I talked to the nurse and she said to just make sure its not getting worse while he is staying up on the every 6 hour Benadryl, so I'll be keeping a close eye on him all day. That is great that your DD was able to have amoxicillin again!

@HappyBaker: Is there any chance he could also have a fever virus causing a rash? Back in July my DD had a fever, ear infection, and rash and the doctor said it could be from an enterovirus and not the antibiotics. She's had another course of amoxicillin since then without the same issue. Here's what hers looked like - very very similar.

ETA: She needed a round of steroids to clear it up.

My DS also developed a rash on his second round of amoxicillin when he was around 8 months. Our pediatrician diagnosed it as an amoxil rash and had us continue with the course of meds. He is now 3 and has had many more rounds of amoxicillin with no issues.

My understanding is that a true allergy is more rare. Might be worth exploring.

Glad you got seen. My middle child somehow developed a tendency to break out in ugly looking hives after colds lately so I would still think it could be viral... but I guess the only real way to know is to see if it happens again with the meds..and I hope you don't find out anytime soon!

We had a similar looking rash, except covering more area. Initially they said it was the amoxicillin, but then it happened several more times when she had viruses, so we eventually decided she’s just prone to getting rashes when she has a virus. With her latest ear infection, I decided to test the theory, and sure enough, the amoxicillin didn’t cause a reaction.

@Kaohinani: Thanks for the tag - you know I like looking at this stuff ;)

I don't think this looks like roseola.. The fever that defervesces sounds good, but the rash is usually more diffuse with less patchy of an appearance.

Could be 5th's disease bc this looks viral, but would usually have same type rash on cheeks (which just isn't shown but maybe is there?)

Could be a heat rash (similar appearance), but those usually don't have associated fever.

My vote is benign viral rash.

@HappyBaker: That looks very similar to my daughter's antibiotic reaction. She is allergic to cefdinir (omnicef) and it took about four days for the reaction to show up (she had been super cranky prior to that). Once we switched antibiotics, it cleared up and she became my happy baby again, which leads me to think that she had been experiencing some discomfort on the inside.

@HappyBaker: @LBee: is right, my son had an amox reaction on his second dose. our ped said that is super common. how many days has he been on the meds? reactions are fairly common toward the end of treatment - my son's reaction was on day 8 of a 10-day course. your son's rash looks very, very similar to my son's - he ended up needing a round of steroids and now has a documented penicillin allergy :bummed:

@Maysprout: Thanks!
@Kaohinani: Thank you, yes it is very angry looking in person!
@LBee: Good to know, my daughter was allergic but it presented itself much more quickly and with her first round of amox, so I thought we were in the clear! Unfortunately I think this guy is also going to be on the ear tube train like his older brother so I'm not sure how we are going to treat all these ear infections in the meantime!
