I have it, also the thin type, diagnosed by an RE during an ultrasound, and I managed to get pregnant twice, successfully. Charting helped immensely since my cycles are long and often erratic.

@snowjewelz: Was she diagnosed through her OB/Gyn?

I was diagnosed with mild insulin resistant PCOS and I had to take metformin. I didn't have any signs of it but my RE checked my insulin levels and noticed a subtle difference.

I have the thin type (however, I do need to lose weight). My first pregnancy I was no insulin resistant so I did there cycles of cloned and one cycle of femera before getting pregnant. I preferred seeing and RE vs. my OB. This time around I was seeing an endo and I was insulin resistant so I was on metformin. Got pregnant quickly and easily.

I wasn't mild, but had great success with acupuncture. I have thin type, which I'm told is harder to treat, but I didn't use fertility drugs for any of my three pregnancies. I just had to do acupuncture and herbs to kick start ovulation as I didn't have periods.
Good luck to your friend!
