@MrsTal: im sorry. That was part of my problem with my previous ob, it wasn't just her, it was mainly her staff. This is hard enough as it is without all of the conflicting answers :( Good luck :goodluck:

@FaithFertility: thank you! I'm so glad to see you around these boards, you give me hope! I saw you finally got some symptoms in the other thread! I am praying for you guys, hope tomorrow's ultrasound goes great!

@owlteach: I actually already switched REs in February, and I love our new doctor but I'm not happy with the staff. I was happier with the staff at the old RE but didn't click with the doctor.. :bummed: it sucks but honestly our old RE didn't find this issue and this one did so I'm thankful for that. I'm planning on having a discussion with my doctor at our appointment next week about the staff issues we've had.

@MrsTal: The inability to give concrete answers is SOOO tough! I think it has easily been the hardest part of all of this. I found that finding a dr. and office that I "clicked" with has helped me to find some peace. Obviously, it hasn't solved our problem yet but I don't spend most of my time questioning the advice we've been given. This whole thing is hard enough, if you aren't happy with your provider, can you find another?

@MrsTal: If I were you, I'd call some various IVF clinics and ask what your chances are with you DH's numbers. I think that might give you some peace of mind. I'm guessing that IVF with ICSI would solve your problem, but I know very little about it.

@gingerbebe: I didn't think to ask if it was Krueger or WHO measurements. Guess it wouldn't matter either way, it's still really low. But thank you for that info, it helps!

@GreenThumb: thank you! That helps. It seems like medical professionals need to get on the same page with the morphology thing.

My re said they like to see 8% for morph. My husband was 4%, but it truly only takes one (which re also said). That being said, IVF with ICSI is often best with low morph. Iui doesn't make much difference (I still did 3, but kinda wish I hadn't.) IVF with ICSI worked.

@MrsTal: DH had low morphology for both SAs (<0.5 first time, and 2.5 the second, all other numbers were normal) and our RE recommended IVF with ICSI. DH did also see a urologist who said that he didn't think that the low morphology warranted IVF, so the morphology thing is still a bit confusing to me, too. But since I also have a blocked tube, we didn't want two things working against us, so we did choose IVF with ICSI and had a successful pregnancy.

Wishing you much luck moving forward!!

@MrsTal: I IMed DH real quick and he said he couldn't remember the type of defect either, but he knows he got a strict morphology score of 3.

I Googled this real quick and it says that the strict Kruger criteria is:

~ strict morphology score over 14%: Normal; fertilizing capacity excellent.
~ strict morphology score 4-14%: Abnormal; possibly decreased or impaired fertilizing capacity.
~ strict morphology score 0-3%: Abnormal; severe impairment or probable inability to fertilize.

@MrsTal: You know, I don't remember what kind of defect it was. DH has that info. I'll try to see if I can track him down. But I know we were in the 90-something percentile range for defects and he also had worse motility than you guys. While he had like 50% motility, I remember him saying that only 15% were actually "good" swimmers. DH used to describe his boys as "lazy, dumb, and slow." Poor guy.

Irony of ironies? We just found we're having a boy (apparently male sperm swim faster, but die sooner than girl sperm??)

And don't worry about how you get pregnant - my only point was that defects can be overcome. We had a few MCs along the way and it took us over 2 years to even get referred to the RE. All told, it will have taken us 3 years to have a baby (20 weeks now).

@MrsTal: We were trying for two years too. The pregnancy was a complete surprise because we weren't trying any longer (I was no longer using OPK's or really tracking anything). We were busy with building and purchasing our new home, and planning to move into IVF with in the next year or two so I completely understand where you are at. We were not giving it any more of our energy when there were other things we could really focus on in the meantime. I found having a plan and knowing where we were eventually headed was freeing after the initial shock and grief wore off. I could take it off my plate of things to worry about and give it to some one else.

Our RE recommended that course of action because then they could pick the best sperm and implant them into the egg. He did a good job of explaining everything. He seemed to think that since that was our only real problem that there would be no reason that IVF was not successful.

@April_Showers: It does help a little. I'm not sure how much longer we can really try naturally, I'm ready to move on and I think we are ready to try IVF w/ ICSI if that is what the next step is. I feel like it is, because I think morphology plays a factor and when only 1% are normally formed it seems like a poor chance. Congratulations on getting pregnant naturally! I can only hope that happens for us, but like I said, after two years it doesn't seem likely.

@spaniellove: That's true it might be. 1% of 31 million is a lot better than 1% of 15 million. That still seems really low to me, in sperm numbers of course.

@alohaorchid: That was what I have been told too. Our first RE didn't even get a morphology report because the other numbers were good. Then we go to our IUI and the RE says that the morphology is bad and that's a big deal. I wish there was just a consensus.

@FliegepilzHut: That's kind of what I was thinking she meant, the numbers in general were better.

@mrs bunchy: I'm not sure, there seems to be a lot of conflicting information out there with morphology. From what I understand anything over 4% is deemed ok, but maybe it has something to do with it? I'm sorry about your miscarriages, I hope you get your sticky baby soon :goodluck:

@mrswin: See I thought our RE felt the same way about morphology because he was saying at our IUI that DH numbers were low enough he'd recommend IVF. I get the same when I look for info on this subject, no real consensus on how morphology impacts IF. If I go with my gut, I feel like this is the answer, this is why we haven't gotten pregnant in two years. I know I'm not a doctor but it makes sense to me that if the heads aren't shaped correctly they can't get into the egg.

@gingerbebe: If you don't mind me asking, what were the numbers and what were the defects (head, tail or midpiece) if you can recall? I want to feel hopeful but after two years it just doesn't feel like there is a possibility of natural pregnancy.

@MrsTal: With our RA my DH's low morphology (0-1%) got us an immediate IVF + ICSII recommendation. Our other numbers were ok (we had a slightly low count but not low enough to be a concern). DH's Urologist said that our other numbers were fine, but he did not really know how morphology played its part. He said there is not a clear answer and no one really knows how important it is. But that lots of couples get pregnant naturally if the other counts are ok, however it can take a little longer. He agreed that IUI might not be our best option though, so we started saving for IVF.

Two months later we got pregnant naturally, so low morphology isn't really the end. It really only does take one good one, and 4.5% of 31 million is still 1.4 million good ones.

Hope this helps a little...

@MrsTal: I know its frustrating but don't get down about it. My DH got similar results when he did his SA. He had good numbers and what not, but his morphology was way off. He kept moping around saying "I'm sorry I have defective sperm" - it was so sad! But like everyone said, the numbers can balance it out. In fact, we got our BFP naturally while we were getting all our diagnostics run at the RE! Hugs and prayers for you.

@MrsTal: I'm glad you were able to get the results. From our RE's perspective very low morphology gets an IVF recommendation right away but a lot of RE's feel that if the other numbers are higher then it will compensate for low morphology.

From my reading there isn't a lot of research or consensus on the impact of morphology on IF.

Since our RE's office staff were being stupid, I drove up to the clinic before work today and got the results from Friday's SA myself. I looked through them and am blown away by what the nurse told us was "better". Overall, his numbers were better, his count was 31 million per mL, for our IUI it was 15 million per mL, viscosity was checked as slight (which is abnormal I guess?), motility was 47% last time was 43%, everything was looking great. But then I looked at the morphology, the one that we were really worried about and saw it was 1% again. The nurse told him is was better than last time but still not normal, so I assumed 3%, but 1% how is that better? 96. 5% were head defects. For our IUI 95% had head defects.

I'm not really sure what to think right now, we have an appointment to discuss our concerns and next steps next week, but I don't know much about this. So I need some resources, I need any information that anyone has on morphology and it's effect on fertility. We want to go into this appointment prepared, having questions prepared, and knowing what we want.
