I think they were usually when my son was no longer eating and just playing around, or if he was frustrated about something. I remember one of the worst bites happened after I weaned him. I had not breastfed him in at least a couple month, and he was really angry about something. I grabbed him to give him a hug, and he leaned forward and bit my nipple through my shirt! He bit really hard too! I remember being so shocked by it, because I was trying to be nice and give him a hug, and he responded by biting me. It sucked- and like you say they are sharp as hell! I would watch her really carefully for a few days, and as soon as she slowed down nursing on one side I would pull her off.
