Lodging was always split between everyone (although we usually didn't let the bride or groom pay). Bride always bought her own plane ticket. At mine, one BM picked up the one night at the hotel, and at others I think whoever was sharing a room with the Bride may have covered the hotel? If the groom to be insisted on Vegas or whatever I think paying his own way to get there would be expected.
