Mine is flexible, I rarely take more than a day or two in a row, my job is hard to miss a full week unless it's planned far in advance. If I want an afternoon/day off (other than sick time) I just ask my boss a few days in advance.

Pretty flexible. We schedule our vacation around the best times to take it. Some weeks are busier than others, but generally, I can take vacation or time off, if I can get the rest of my work done around it.

I give notice as I know that I want the time off. Like I scheduled Christmas/New Years in February. But I just take off when I can.

I am not allowed vacation time during the first 9 working days of the month.

It's flexible. I try to give as much advance notice as I can, but it doesn't really matter that much. I just tell my boss and as long as he's okay with it (which he always is), that's that.

We have a team website through our intranet at work and we all put down our PTO on the calendar on that site. We don’t have to ask permission for take time, just put it on the calendar so everyone knows when people will be out.

ETA: My company (and department) are very supportive of using your time, so it's totally fine to take a day/time off when others have already put themselves down on the calendar for that day. As long as there will be 1 team member in that day, everyone else could take the same day off and no one would mind.

I'm fairly flexible around publication dates/deadlines for work that I am involved in (some projects are monthly, some are bimonthly, and some are annual), but beyond those dates, I can book time off whenever.

ETA: DH on the other hand has to schedule in advance. And this year, he asked about our usual vacation week in August (when my whole family goes to my parent's place), and was told he needed to wait to book it off to see who else was around. Well when it came up again, his coworker said they'd already booked something, so now he's not allowed to take it off! Even though he'd brought it up before her and she just went ahead and booked without getting approval. He's pretty ticked about that.

We make up the on call schedule quarterly so I have to ask 3-4 months on advance if I'll be off on a day I am on call. It's a huge pain.

I'm a college professor, so we schedule vacations to coincide with school vacations or summer break. DH's work is flexible. He just has to put in a time-off request in advance and try to avoid conflicting with big deadlines... although sometimes the deadlines crop up unexpectedly at the last minute.

DH has to request it off a few weeks in advance or else he risks someone senior to him requesting the same time off first.

I make my own schedule so I don't have to get permission.

I just submit timeoff whenever I need it since normally it's just a day here and there. We always take a week off during Christmas so my boss expects that annually. So glad work is flexible!!

We're similar to you. DH has to put in his loose schedule within the first quarter. I will put in for long vacations (1 week +) pretty early on, but then will ask for a vacation day a few days to a week in advance on the fly unless I know about it sooner. We have a request system that my manager approves. I've never really been denied for any vacation time.

I mean they say they want us to schedule asap, but my boss knows that sometimes you just can't so he's more or less flexible. My boss is traveling right now and I still haven't requested 1 week I need off in July!

They just implemented a new program on the computer that allows us to view our schedule and also request time off. They ask for 6 weeks notice but you can request off even just 2-3 days in advance. The computer program will view staffing and determine almost immediately if there is availabilty to lose staff for the day.

DH has to put in his annual vacation at the start of 1st quarter! It's flexible-ish, but typically we just end up going with his schedule as our guideline.

Mine's on the fly as we need to take off... but typically for 1 week+ vacas we try to give a few weeks advance notice
