I can't find it in my policy right now but the language in my policy only addressed that injectibles/IVF/GIFT/ZIFT were not covered = infertility not covered at all. So we didn't realize for a long time that actually all testing, monitoring, and IUI were totally covered. I talked to someone at my insurance company and they gave me such wrong information. A quick 5 minute meeting with the finance person at the RE cleared it up, and way more was covered than the insurance company led me to believe. It may be that the "low tech" stuff isn't specifically mentioned but is still covered. My Infertility coverage policy (which I read top to bottom and still didn't understand) made it sound like 6 cycles of ART were covered, but what wasn't clear is that IUIs (and FETs) aren't considered ART, only fresh IVF. That didn't mean that IUIs and FETs aren't covered, just that they're not counted toward the lifetime max of 6 cycles. Our plan on paper makes it sound like nothing is covered which worried us, and actually our first clinic billed us a $40 specialist copay for each monitoring visit because no one knew what was going on.
