I had the same question too because sometimes he just doesn't seem hungry. I still follow the advice from the baby whisperer book which says 2.5 to 3 hours. Then we went to the lactation consultant asking the same question, and she said if your baby doesn't want to eat or isn't hungry, it is ok to go 3-4 hours. At night we do not wake him to eat - if he wakes then I feed him 1 time. It's definitely hard to tell whether my LO is hungry or not because he's also been sucking on his hands a whole lot (oral stage). I also notice some days he seems to be content going longer hours (3-4 hours) before the next feeding. They get some bad raps but they have the kind of information you are looking for. As far as his weight as long as he keeps on a curve, he is fine. Our little guy went from the 50% to the 0.5% so we freaked out. My mom just said, "when he's hungry" - well d'oh! But sometimes he doesn't show signs of hunger, or he takes long naps or we're out and about and it's been longer than I realize and he was content to sleep/watch/observe. He has gone as long as 4 hours during the day between feedings, but usually I TRY to feed him ever 2-3 hours. That's how often I fed him as a NB though. How often do you feed your 4 mos old?
