Usually within 15 but they always say to call back if no one calls within 30. Recently dd spiked a crazy high fever in the motn, 104.2. First thing we did was give ibuprofen, place cool cloth over her forehead then call pediatrician. They said to also put her in a lukewarm/cool bath; not cold but not hot. If that doesn't bring her temp down then call again.

Ours always calls within 10-15 minutes but I've only called a couple times. It probably varies. Glad they got back to you! High fevers are concerning but particularly if they don't respond to treatment. Our kids' fevers have always gone down faster with ibuprofen. Check again but if it goes down (sounds like it already has) I think you can wait till morning and see how she's doing. Like a PP mentioned, a cool wet washcloth wiped over baby's face/head always seems to soothe them when their fever gets so high like that.

@tiramisu: 20 months, so not that tiny. Seems to be getting cooler, just sweaty. Poor kid- hope it isn't another ear infection.

@Charm54: they did call me back and said to wait until the morning and reassess- but she hasn't been on antibiotics or anything and it just started. I'm going to check on her in a little bit but seems like the motrin is working. Still seems like a longn time to wait for the call but I did like this ped.

I've only called the after hours number once, but the doctor called me back within a few minutes.
How old is your DD? If she's really young they may want you to bring her into the ER since her fever is a little high.
In the meantime I'd keep checking to see if she's responding to the Motrin. I've also used a cool wet wash cloth on my daughter's head and body to help bring her temperature down. Hope your LO feels better soon! If doctor doesn't call back soon, I'd call again.

Hmm our doctor doesn't have an after hours number....when dd had a fever of 39.9C at 1am
a few months ago, I called our province's nurse line who told me to bring her to the ER. C had an ear infection, was on antibiotics that weren't working, and Tylenol/Advil wasn't bringing it down so that's why the nurse advised that we bring her to the ER rather than wait for our family doctor in the morning.

I would check her in an hour or so and see if the fever is going down after the medicine. Poor little thing! I hope she feels better soon!

C spiked a fever shortly before bed- it had been a little over 100 this morning, but went up to 103.9 right before bedtime. She's been cranky but otherwise fine all day, so I'm not overly concerned, but I called the ped office just in case.

I called almost half an hour ago. I mean, I'm expecting her to say that as long as she is acting ok and responds to motrin it's fine, but seems like she should have called back by now. Right?

Also, for real, a fever hovering just under 104, along with a cold and crankiness, is nothing to be crazy worried about right? I had just given her Motrin when I noticed how hot she was. Will check her in a little bit in her crib.
