<description>&#60;p&#62;M was on nutramigen and our insurance covered it.  I had to fax them a prescription from the pedi and then find a medical supply company (I think the insurance company website had a list of companies) to actually send us the formula.  They contacted the insurance company and got approval and started sending us the formula.  Every month I would call the medical supply company, they would get re-approval from my insurance company and send the formula.  &#60;/p&#62;
&#60;p&#62;I was shocked that our (not so great) insurance company covered the formula 100%.  When I called to get her refill for September they told me that our insurance company refused to pay.  After a few weeks of phone calls the company told me that they had a new policy and would need repeat visits with a GI specialist, an allergist and a nutritionist and documented evidence that we had tried a traditional formula and failed every few months.  Luckily M turned 1 at the end of September and did fine with cows milk.  I was literally on the last can of Nutramigen and didn't have to buy any!  I think they were just going to make us jump through hoops because they didn't want to pay for it.
