@loveisstrange: thank you! Will look those up. @MenagerieMama: thank you!

Thank you to all! I'm sure some of you seasoned mom HBers are like pssshhht FTM with a first rash! Ha! Which is probably true. And this is totally low on the Things That Really Suck list. But your replies have been super helpful. It's distressing when there are/could be a zillion things causing something and a zillion things that could be used to fix it! So hit or miss and frustrating to find the right one that works!!

Nystatin never worked for us. :/ My DD was prone to angry, bleeding rashes any time she was teething.

I had the best luck with two things. One being titanium dioxide cream (we lived in the UK and it was readily available, but you can order it on Amazon). The brand is called Metanium. It smells vile. It's bright yellow and can stain.... but it works soooo well. http://www.amazon.com/Metanium-Nappy-Rash-Ointment-30g/dp/B000KUALAY

The other was homemade oil based stuff. I bought from this lady and we still have jars hanging around that we use on dry skin and such. Vanilla Cake smells amazing. https://www.etsy.com/shop/PattycakesBalms

@Littlebit7: just read one of your updates - if it's really red around the anus maybe have it swabbed for strep butt. It's a thing! Antibiotics would help then if it was strep.

Okay I didn't read most of the replies but here are my major points:
- baking soda baths after any runny poops
- diaper free time
- air dry all the way before fresh cream/diaper
- lots of aquaphor! I bought every cream known to target after LOs first bad rash (from runny poops) and Aquaphor was still the best
- if the red is in the creases get OTC lotrimin cream
- use cloth wipes with water instead of baby wipes

Good luck!

@BabyBoecksMom: tried that. It actually looked a bit worse after using it.

also i'm using prescription nystatin for a yeast infection (she also gave me a steroid cream but I've have been hesitant to use that) I have been using the nystatin since last friday. Thats exactly how it started, little red dots! And when she pooped this time its like the same red dots just got SUPER angry again.
I'm headed to the doc tomorrow.. Hopefully I'll have some answers or maybe just be told to ride it out (which I'm ok with doing as long as I'm riding in the right direction!!

We love Triple Paste, and we have a prescription cream that also works wonders - we switch back and forth between the two during the day and it usually clears up within 48 hours.

As a side note: Do you think it could be a yeast infection (starting out as little red dots)? If so, you will need to put a layer of Lotramin cream on it (athlete's foot cream) and then cover with a thick, zinc oxide-based cream, like Triple Paste.

@MrsSRS: she was eating either oatmeal or brown rice cereal with apple sauce mixed in. toast. a pouch with banana mixed with pumpkin. that was for about 4 or 5 days

yesterday she had toast with cream cheese on it and some plain chicken thighs in addition to plain oatmeal. ah and some blueberries? Maybe those were acidic? her formula is dairy based Holle. Introducing one thing is what I should have done but my brain wasn't registering this could be a food sensitivity. :bummed:

update!! rash was slightly better for 2 days because her poops were less frequent (and no waking up with a poop diaper). I had also implemented a pretty plain BRAT diet, nystatin from the MD who thought it was fungal, baking soda soaked paper towel towels and bath, and SLATHERED A&D (I haven't stopped nystatin) So I started to get back to her normal diet yesterday. BAM. frequent green poops again and little tiny red dots, which today became larger angrier red dots and a rash/ring around anus. UGH. So now I'm thinking its acidic poops from food? Reaction? Allergy? Literally she pooped, I changed the diaper 4 min later and the angry red dots were there.
We're going back to the ped tomorrow. She has ZERO other symptoms which makes me wonder if its a food allergy. Oh and still drooling from two teeth.
So back on to bland diet. She had a little dairy yesterday (Cream cheese on toast), but she's never had a problem with dairy. She's formula fed for the past 8 weeks.

UGH. I'm going on two weeks of this! Tell me it can take this long to get a handle on things?

One other idea if nothing else works: LO only had very bad diaper rash once, and it turned out to be from a bad pack of diapers. She's used the same diapers from birth, and all the sudden she had a bad rash. Nothing worked for a few days, but when a weekend hit I grabbed a pack of diapers from a new box and used only those for Sat/Sun. Rash immediately cleared up. I didn't think anything of it, until she went to daycare on Monday and came home with a bad rash again, which got better overnight. Same on Tuesday, so I sent in a new package of diapers there, and no problems since. I honestly think I got a bad batch of diapers for some reason.

(ps. I returned the bad box to Target and they gave me a full refund even though it was partly used!)

@Jess1483: it's no worse! Slightly better I think? I've been doing a&d during the daytime with a teensy bit of desitin where the rash is the worst. At night a&d and lots of max strength desitin. Baking soda in a bowl of water where I soak her wipes and baking soda in the bath. Diaper changes every two hours and naked time. She's not screaming now! So that's great!! Still a lot of pooping though....
Thank you for all of the advice!

for every diaper change we use A&D ointment (not the white) and cornstarch powder

if he has a rash(usually does with his sensitive skin) we add Paladin http://www.familyotc.com/Paladin-Diaper-Rash-Jar-Ointment-2oz-p/cd2167187.htm

and then do A&D and powder over it

I've used every cream and the Paladin works the best, we discovered it when my older LO had runny poops for a month and were desperate

Wipes always seem to exacerbate my daughter's rash and it's the first thing the nurse line tells us-ditch the wipes. When it's really bad we do the sink and naked bottom as much as possible but we have permanently switched to water wipes and it has really helped. Most Targets carry them.

We recently had a pretty bad rash too. This worked -

NO wipes, not even cloth w/ water. Rinse her butt. Her we squirted water at her butt and gently pat.

Then let her hang out to air dry her butt. We don't want her to walk around so we put on a video and just let her lay there.

We put on straight up cornstarch i/o baby powder.

Then LAYER on whatever cream (Desitin works fine in our case), and then layer on A+D, thick thick thick. We don't put her diaper on too tight, and if she's wearing a onesie I don't button it to tighten that area.

Our LO reacts really badly to all the diaper creams that everyone usually recommends (like Butt Paste, etc). Her rashes just get worse and worse really quickly when we use them. We use plain A&D ointment and that's it. I know we are a special case, but just wanted to chime in in case your LO is sensitive to the zinc ones like ours is.

We were also using cloth diapers when she first started getting rashes and nothing I did could stop them. We switched to disposable (which I previously thought were worse for rashes) and they cleared right up.

@T.H.O.U.: I tend to agree about using too much zinc cream - I think it irritates LO's bum. We have found the Motherlove Diaper Rash & Thrush ointment helps to heal/prevent anything that may be becoming a yeast rash. It seems like a small container for the price, but you only have to use a little and it goes a long way. We apply that and often put Aquaphor on top as a barrier.

I know Target carries it - not sure where else

@T.H.O.U.: ahh interesting. I always do a layer of a and d for every change since she was a newborn. So maybe I'll just tweak that and add the zinc at night. Do you just layer it up? Mix it all together? I had read previously that doing of lots of higher zinc context creams can make things worse/cause irritation. So I like the day/night idea

From a mom at daycare:

our pediatrician has quite the theory about diaper rash that I thought might be helpful to share! He says that during the day, rashes should be lathered with aquaphor or something similar, which creates a moisture barrier between the skin and the diaper and also helps ease any burn they might feel from urine on the rash. Actual diaper cream should only be applied at night and it should have the active ingredient of zinc oxide, which is the drying/healing agent. He thinks that too much of that one (day and night) actually can make the rashes worse by irritating them.

* We love the Red Butt Paste at night. Very high % of Zinc Oxide.

@Littlebit7: not your fault! These things happen. DD1 was never prone to diaper rashes. But Dd2 gets one probably every few weeks. If she poops and we don't know and it sits for more than 10-15? She gets a rash. She had a really bad or around that age and I was furiously googling natural ways to cure rashes. Baking soda came up. And well, it's baking soda, I figured it couldn't hurt. Baking soda is awesome! You can literally use it for everything! LOL

@regberadaisy: oh thanks! The baking soda is interesting. Like that idea will put it in her bath! I don't think it's the diapers at least I hope not. She's been in Swaddlers for 8 months. But I will keep it in mind if other methods don't work.

This is totally my fault. I made a comment the other day that she hadn't gotten a diaper rash yet....so yeah. :bummed:

My foolproof method for bad diaper rashes:

no wipes, just use a washcloth and dab. use warm water with baking soda
air dry!!!
Layer of coconut oil then thick layer of desitin rapid relief
warm baths with baking soda

baking soda helps to neutralize the acidity of the runny/diarrhea poops.

Poor baby, hope she feels better soon! Also, Pampers gave my girls rashes, so a different diaper is def worth a try.
