Dr. Google says if it were a cold sore more than likely he'd have a fever and more than one spot (because its the first time). He has no fever and just one bump, which doesn't seem to bother him at all! You can't get cold sores on your breasts, same way you can't get genital herpes on your fingers or feet or whatever.
