Funny how we can plan all we want and Mother Nature takes its own course. I had day 3 check in today and a big cyst on my ovary. No clomid for me this month :( We have to wait it out until it goes away. To say I'm bummed is an understatement. On to hoping this thing shrinks in a reasonable amount of time...

@Madison43: We're trying to find that balance. I wouldn't say I'm resistant to clomid, but it's so uncertain whether I'll actually ovulate or not. My RE is hesitant regarding the injectables... time will tell on the next few cycles what we need to do induction wise....

I'm 26 - I understand it may only increase our chances a little, but I'd rather do something a little more rather than keep doing what we're doing! We're going to ask for the IUI at our next appointment. Our RE is really great about closely monitoring, so I feel comfortable about that :)

I guess it depends on how you are responding to the Clomid? I have PCOS and after 2 rounds, became clomid/femera resistant. I moved on to injectables, but the dose necessary for to ovulate would have produced multiple eggs so IVF it was!

If we had the chance, I would have done IUI's before IVF. But unfortunately we had the PCOS/anovulation issues PLUS dodgy male factor issues, so the FS told us our chances of conception from IUI were basically nil. If it's covered through your insurance and his swimmers are good? I'd TOTALLY do IUI.

What's your age? That would make a difference for me as to how soon I'd move on to IVF. If you're relatively young and have been screened and found to have no tubal or other issues that would require IVF for success, I would definitely start with a series of (closely monitored) clomid or injectable IUI cycles. So great that you have some insurance coverage!!!

I have PCOS and basically don't ovulate either. I also am Clomid-resistant, which is common in women with PCOS. What worked for me was the highest dose of Femara + Gonal-f at the end + trigger shot, with timed intercourse.

I imagine your RE is not telling you that IUI wouldn't work, I think they were probably trying to say that in your case it's not necessary. IUI only increases your odds very little if you don't have any male factor issues.

I'd say that if you're tired of waiting around, then go for the IUI. Make sure to get monitored and to use a trigger shot. If you have any questions I'm happy to answer :).

@Sunshine1810: yes!! congratulations!!
@sweetooth: that logic definitely makes sense in terms of DH ;)

Thank yall so much. I appreciate the insight. I thought it was strange to kind of "skip over" the IUI procedure, but also didn't want to knock her "recommendation" of doing so.

There is not an insurance pre-req to go straight to the IVF; however, based on the comments today, I'll ask for the trigger and IUI combo this next go around and see what happens!! I appreciate the thorough feedback

What @sweetooth: said. "Super swimmers" has nothing to do with conception. My husband has "super" ones too and I have no known issues and we've done Clomid with IUI. From how my RE explained it, why do Clomid alone when there's more you could be doing? I would try that with IUI, since insurance covers it, before moving to IVF, if necessary. Do you know if your insurance requires anything pre-IVF? Mine requires a set number of medicated cycles, with or without IUI, before moving onto IVF.

If I were in your situation, I would probably go for at least 1-2 IUIs because they are covered. I have PCOS & DOR and while my DH had history of low morphology, the cycles we did an IUI his numbers were good. Unfortunately, none of my 6 IUIs were successful. (First 3 IUIs were at another clinic with unknown diagnosis and last 3 IUIs were at new clinic with a diagnosis) We, also, have no IF coverage, so IUIs were all out of pocket and cheaper than IVF. We *thought* IUIs would have worked. Once I got my PCOS & DOR diagnosis, the RE did say I could go straight to IVF and skip the IUIs. My reasons for going for last set of IUIs...
1) cheaper than IVF,
2) RE was hopeful that I could get pregnant with the new meds and with IUI and
3) For the future, we wanted to know our chances of an successful IUI. If we went straight to IVF, we would have never known if IUI would have been successful. This is important to us if we want to have a second child.

Given all that I said, if my insurance covered it and that fact that I want to get pregnant like yesterday. Oh, and knowing the 6 IUIs were unsuccessful. I would have liked being able to move forward with IVF. I would probably would be holding a child right now in my arms.

@sweetooth: Thank you! :) It feels so strange to finally be able to say that!

Also, @sweetooth is right. IUIs aren't nearly as hard on the body as IVF, so I think DH and I wanted to make sure we exhausted all our options before going to IVF.

I don't understand why your husband having "super swimmers" would mean that IUI wouldn't work? That confuses me. It seems like "super swimmers" would be great because they IUI would put those swimmers right where they need to be, right at the right time (with monitoring), and then the magic takes it's course.

We have had success with IUI. I conceived on a injectables/IUI cycle. Clomid was thinning my lining. We have unexplained infertility.

If insurance covers it, and you're up for trying a few IUIs, I'd recommend starting there. I agree with @twodoghouse about the emotional and physical toll it can take on your body. IUI failure isn't fun, but it's not as hard on the body as the IVF process (in my opinion...I'm not trying to offend anyone who has been through IVF)

I think the monitoring is the most impt thing whether you do Iui or not, you need something to help you with the timing. My O date got shorter with each Clomid cycle. If Iui was covered and you aren't in a big hurry...try it out. In some respects, you can only count the months you've ovulated as "real" TTC. And just based on odds of the right sperm and egg, it can take awhile. My baby is any IVF baby and we did 3 iuis, but had more male factor issues and no pcos.

We skipped IUI and went straight to IVF. Worked the first time! However, since you have IUIs available paid by insurance, I would do one or two before going to IVF. IVF is so hard (emotionally, physically) that I would try other things before going there. Good luck!

I have PCOS as well. If you have coverage and you respond to Clomid then maybe just try a couple IUIs before moving on to IVF? IUIs were not successful for us, but I just got pregnant from our second round of IVF. I have seen a lot of people have success with IUIs though! Good luck! :goodluck:

I should also add that we did 3 IUIs before moving on to IVF. Our doctor was okay with going to IVF whenever we were, but we just weren't ready yet without giving IUIs at least a shot.

It sounds like you only had 2 "real" tries where you ovulated. If it were me, I would give clomid another try or two (maybe with IUI since it's covered anyways) before getting more aggressive. DHs numbers were great and she didn't think IUI would help, but it was covered for us as well so we figured why not. I'll take any microscopic better chances I can get. What about FSH/IUI? We gave all the "easy" stuff a go before moving to the next step.

@pui: Good point - of all the attempts with Clomid, only 1 time it did not work. I don't chart - I work crazy hours sometimes with travel so if I get a chance to sleep in, I'm taking it  It would be difficult to catch the same time frame each day.
We didn't discuss the trigger shot yet. That's a good idea and I'll bring that up at our next appointment!

Have all your attempts at Clomid made you ovulate? If so I'd probably try Clomid a few more times. Maybe try a different ovulation medication like Femera?

Do you chart? How has your timing been on your Clomid cycles?

I also have PCOS and didn't ovulate on my own most of the time, but the rest of my and DH's reproductive systems looked good. The treatment I was recommended was Clomid + monitoring then a shot of Ovidrel and timed intercourse. After a few cycles like that we would have tried some IUI cycles before considering IVF.

If your DH's swimmers are good then yeah in theory an IUI is unnecessary, but I'd still try a couple cycles before moving to IVF.

Hello! A quick background on myself.... DH and I started TTC even before our wedding because we knew we would have a hard time given my medical history (PCOS, ovarian cyst removals all in my teenage years). This was 3 years ago. We tried for a year, did 2 unsuccessful rounds of clomid, and decided to take a break from the treatment. We continued to TTC on our own and now seeing an RE again. So, 3 years with off/on infertility treatment. We did all the testing and I just don't ovulate - everything else looks great.

We started seeing a new RE and she's very open with whatever course of treatment we decide to take. After another failed clomid (ovulated but no conception) round, we want to kick it up a notch and get more aggressive with the treatment.

I thought IUI would be a logical next step, but the RE recommended against it since DH has super swimmers. Thoughts?!? She's more than willing to let us try it, but she warned she didn't think it would be successful. Based on her advice, we either continue the clomid or go for the IVF.

I'd love to hear your thoughts or suggestions for other options we might have no considered at this point that worked for you...... Also to note, we have 6 IUI's paid for and 1 round of IVF through insurance. Super thankful for that!!
