My oldest went tumbling down when he was 3 missing a step on the way down. I think a gate at the bottom would have helped soften the blow to his head, maybe... Luckily, no concussion but the sleepiness and yawning 5 mins later was really scary... My youngest LOVED to climb up the stairs... a gate would have been nice.
