$6,000 deductible then we pay 20%&#60;br /&#62;
I don't remember the OOP max... I want to say like 9k or something.

<description>&#60;p&#62;Mine is actually an HRA (not HSA, which means the funds aren't portable and mine to take with me, they are my employers to keep if unused).&#60;/p&#62;
&#60;p&#62;Employee only coverage:&#60;/p&#62;
&#60;p&#62;Premium = 66.00/month&#60;br /&#62;
Network is extremely narrow it's one hospital group and their affiliated providers.&#60;br /&#62;
Deductible = $3750.00&#60;br /&#62;
OOP Max = $5350.00&#60;br /&#62;
Coinsurance = 5% after deductible&#60;br /&#62;
Contribution to HRA $1250&#60;/p&#62;
&#60;p&#62;For the family coverage (spouses aren't covered unless you can prove they don't have coverage through their employer, are unemployed, self employed, etc.)&#60;/p&#62;
&#60;p&#62;Premiums = $484.00/month&#60;br /&#62;
Deductible = $7,500&#60;br /&#62;
OOP Max = $10,700&#60;br /&#62;
Same HRA contribution.&#60;/p&#62;
&#60;p&#62;Our out of network coverage isn't significantly worse, because the in-network providers are sort of HMO-like in that it's just one hospital network.

&#60;p&#62;Luckily, they don't put a cap on HSA roll-over.  DH was on this plan as an individual for several years without any major personal medical expenses, so by the time the rest of the family joined we had quite a bit stored up in the HSA.  The kids used to be on my PPO plan, but I was laid off last summer and work in a contract position now.

&#60;p&#62;The summary of benefits estimates a simple pregnancy and delivery at $6030. &#60;/p&#62;
&#60;p&#62;This plan basically has the maximum allowable customer cost-sharing, and what makes it even worse is the network. It has a pathetically narrow network yet going out of network would almost double my cost (i.e. coverage diminishes to practically nothing). &#60;/p&#62;
&#60;p&#62;I am in a state with continuity of care regulation that requires the insurance company to keep paying at the in-network benefit level if your coverage changes in the second or third trimester (this is to avoid forcing you to change during pregnancy), yet they've done nothing but give me a hard time for the last three months over my midwife being out-of-network, trying to get out of their responsibility. It is SO stressful. Their network has ZERO midwives within 100 miles; I used to have a choice of over a dozen. Several local hospitals, including the first choice for VBACS, are not in-network either. &#60;/p&#62;
&#60;p&#62;I feel like the insurance company is just taking advantage of us in every way.

&#60;p&#62;$240 a month for family premiums&#60;br /&#62;
$1000 dollar employer contribution, with a $500 bonus if you max it out&#60;br /&#62;
$4000 deductible&#60;br /&#62;
$8000 OOP max (I think?)&#60;/p&#62;
&#60;p&#62;Which is waaaay better than the plan I was receiving through my employer and I am a teacher! I even chose the lowest level plan with the lowest premiums.&#60;/p&#62;
&#60;p&#62;$356 a month for just my son and I. It would have been around $700-800 for the whole family&#60;br /&#62;
No HSA, just an optional FSA&#60;br /&#62;
$5000 deductible&#60;br /&#62;
I can't remember the OOP max, but it was really high
