Nothing. Part of the reason we decided not to pursue anything beyond Clomid and went straight to adoption.

I haven't checked, but I'm pretty sure that mine covers nothing. My insurance is through my work and it is for a religious affiliated school.

insurance covered all my infertility testing and chromosome testing, but nothing else. Clomid was at-cost ($20/month) and now i'm on Folgard, which they cover for $5/month. I guess there are lots of reasons women take that medication, though.

My state does not require insurance to cover fertility stuff, though. You are very lucky if it's covered!

We were so lucky, we could get up to $10,000 for IVF and everything before that (diagnosis, meds, IUI, etc) was completely covered after the co-pay of $20/visit...so thankful for everything!

Mine has proven to be better than I anticipated. It covers diagnosis and treatment of the root cause of IF. It does not cover IUI or IVF. It covered my Clomid, Femara and all the monitoring ultrasounds. So far, they have covered my first round of injectible medications and I am hoping they will approve the ultrasound and bloodwork monitoring visits. We will pay for IUI out of pocket but I hope all the steps leading up to the actual IUI procedure are paid. We'll see.

As others have said, my health insurance covered diagnosis for me and an initial SA for my husband, but the actual IVF itself was paid for out-of-pocket. We, however, have separate health and prescription insurance, and a lot of my medication was actually covered in part by our prescription insurance. That was a HUGE help!

My insurance covers nothing - diagnosis, but no treatments. We got pregnant via a free IUI study at a teaching hospital near us. I don't know anyone in my state that has infertility coverage.

DH's employer is in NJ, though, so I need to look into what his coverage entails - we may need IUI or IVF to get pregnant a 2nd time, or we could end up getting pregnant no problem...we'll see.

My insurance has a lifetime maximum of $25,000 which included IVF. I called my insurance company and got the discounted rates that my RE's office accepted to help figure out how many rounds of IUI/IVF we could afford. My medications came out of my prescription coverage. I'm lucky that I also have insurance through my husband. His insurance gives us a little less and any medications come out of the infertility coverage. But we were able to just use my insurance so far.

Errrr. I'm learning this the hard way right now. My ins is great provided you are healthy or need tons of care, but anything in the middle and it blows! So it won't cover any testing or treatment for anything until you've met your deductible but once you've met it then it pretty much covers everything (ma requires all private health ins to cover ivf, so it's more that my state has provided reasonable minimum coverage requirements and our ins covers those to a T) minus your copays which depend on if you stay with preferred providers or not. So if you need expensive treatments, you won't go bankrupt from them, but if you need a few labs and minor help it'll probably cost you the same out of pocket as the person getting $15,000 in treatments.

Short version: it covers anything once I've met our deductible, but there are copays and no diagnostics or treatments are coveted until deductible is met.

Ours covers everything until IVF, but no meds. And I'm paying a $40 copay at each monitoring appointment so that adds up pretty quickly but it beats not having any coverage.

We have a lifetime max of 10k. It covers treatments and testing except for IVF. Luckily the drugs were covered under our prescription coverage. I need to find out how much we used. For #2 we might have to pay out of pocket.

I think I have pretty good insurance but in terms of FT, it only covers 3 IUIs. If I have to go to IVF, I think it's out-of-pocket.
