Another pediatrician here. I totally agree with @sproutbee. The concern is the discrepancy between height and weight percentiles and the association with high BMI as a toddler with obesity at a later age. I would try not to take offence. It's good to know about the potential concern so you can keep an eye on his percentiles and emphasize healthy eating, exercise, and low screen time. If you're already doing all those things that's great and there is a better chance of his percentiles evening out down the road.

LO is 96 percentile for weight and 98 for height. Our Dr has never seemed concerned but we also are dairy free because LO is intolerant so she already drinks lo calorie milk like flax and and almond.

@blackbird: Food log is a great idea. I know I sometimes mindlessly give LO some crackers or a pouch when we're out and about to keep her quiet and I forget later when I'm thinking about what she ate that day.

My older DS has been 90%+ in weight. Before he was two he weighed 37 lbs, more than a lot of school age kids. I'm on the small side and DH is average so we were a little worried but our ped was totally unfazed and didn't think it was anything to be concerned about. If you want to switch to 1-2% milk because that's what you and DH drink, do it. If you don't want to, don't worry about it. I don't think that will matter much since he barely drinks any. I would say don't stress about it even though I know that's hard.

So E was 80th for weight and 60th for height at her 2 year appt and her ped said not to switch from whole milk to skim. He said skim its altered more - and actually has more sugar. He said the fat is fine but it's the carby meals and snacks which add up on their little bodies. It's hard because E was a very picky eater...she would only eat toast for breakfast, crackers, cheese and berries for a snack and some type of sandwich and pasta for dinner. Luckily, she outgrew some of that pickiness. I just started offering her more fruit for snacks instead of cheddar bunnies. She loves veggies now which helps. Also, between 2-3, she started moving more - she wouldn't fall so often and wasn't as cautious as she was when she was 1, so now she runs all over the place.

Her weight and height are now even at the 65th percentile.

Hi. Primary care pediatrician here. The concern is the discrepancy between height (40th%) and weight (90th%). BMI can be calculated at that age: http://nccd.cdc.gov/dnpabmi/Calculator.aspx. There is growing concern in the medical community that a high BMI at a young age - even as young as 2 (some think even younger!) - can increase your risk of continuing to have a high BMI as an adult. There is definitely no *one* fix for all families, but I would take a close look at your child's intake (both quality and quantity) and activity level and make adjustments where you can.

The whole idea that fat makes you fat has been disproven. Low fat milks have more sugar in them- I'd keep your child on whole milk. http://www.npr.org/sections/thesalt/2014/02/12/275376259/the-full-fat-paradox-whole-milk-may-keep-us-lean

at her 2 year appt, my daughter was 80% for weight and 75% for height. She went up from 50% weight from 18 months but her height percentage stayed the same so her ped recommended cutting out snacks and whole milk. I think that's just the cookie cutter recommendation. But my daughter BARELY drinks milk and I'd rather not give her skim/low fat since I'm not comfortable with the whole process of cutting out the fat from milk and it's not adding to her weight. She does snack, but her snacks aren't starchy either ... so again ... sometimes recommendations are very cookie cutter based on what is the most common factor. At least in my opinion. At her 3 year check up, she was back up to 85% for height and 75% for weight ... so ... bleh. I take it with a grain of salt. She's always been taller than most kids and taller generally means she weighs more. So I'm not too stressed.

I definitely wouldn't do skim! I'd probably do 2% but like you said if she doesn't drink much milk I don't think it will make a huge difference. I would actually be pretty offended at that suggestion. It's like he's suggesting to put your 2 year old on a diet. No thanks!!

@lady grey: That is the general recommendation in Canada at least, for toddler/preschool ages. Doesn't have to all be strenuous activity though. Some days are hard but I do find the days we meet or exceed that bedtimes go much better!

I personally believe WM is better for younger kids (for the same reasons others touched on) and wouldn't do skim. We actually don't drink milk at all ourselves though so the only milk in the house is for her. If his diet is as healthy as you say, I wouldn't worry about it--just make sure his portions and exercise are appropriate (and you sound like you know what you're doing to figure it out).

@rachiecakes: @oliviaoblivia: I think you guys are right, it just really caught me off guard since I don't think my son looks that short. No one has ever commented to me that he looks short, so I didn't expect such a discrepancy.

@reverie: @regberadaisy: I think we will switch to 2% that seems like the right thing to do. I feel like skim milk is pointless...might as well stop giving him milk and just do yogurt, etc.

@Mamasig: @lamariniere: exactly! my LO is not a chubby kid! he doens't look that big, but is very solid.

@californiadreams: I may look for a dietician if its still a concern at his next appointment. I do feel like the pediatrician was pretty rushed and did not do a full assesment of our lifestyle and LO's daily intake. He just saw the height/weight numbers and told me to go to skim milk.

@Tanjowen: thank you, that does make me feel better. I know he eats lots of healthy food and is very active. I will now make even more of an effort to make sure he is getting lots of chances to be physically active every day!

@daniellemybelle: I will prob give this new ped one more chance, and if I don't get a good feeling from him next time then I will probably switch. I loved our last pediatrician so this was a pretty sad experience for me, seeing someone who was so rushed and gave me such intense recommendations!

Also 1 & 2% is supposed to be better for heart health. That's the primary reason why we switched our daughter at 2. We have a history of heart problems on both sides. But DD1 is on soy so it's a moot point for us anyway now!

@vnvdvci: thanks for your thoughts- curious, where did the recommendation of 180 minutes of physical activity per day come from? I'd like to be able to research that further.

I would switch to 1 or 2% but I would not do skim milk for any children. They do need some fats in their diet for brain development and milk is one place they can get it consistently.

I am pretty picky about pediatricians and I would not have loved that he recommended skim milk for a 2 year old and didn't even ask you about his milk intake or what his regular diet is. It is not one size fits all. So, if I were you I'd probably keep shopping around but I am always shopping around for the perfect pediatrician that has all the AAP guidelines memorized ;)

My toddler is in the 90th percentile for weight, but the doctor isn't concerned because his height is 98th percentile. I think they get concerned when there is a big difference between the height and weight percentile. In your case, I would probably do what @Anagram suggests and look at the overall diet. Maybe you could cut a snack out or try giving more fruits/vegetables instead of carbs.

My son has always been in the 90+ percentile for both height and weight and the pediatrician hasn't been concerned. She always just says to keep doing what we're doing because it's obviously working. She did say we could switch to 2% at age 2 if we wanted but we chose to keep him on whole milk. The calories are not all that different and we like that whole milk is less processed.

i would consult with a registered dietitian over your ped if you are truly concerned. But then, I would not be concerned and I certainly would not introduce diet products to a child. Children are born as intuitive eaters and he will eat what his body needs. I wouldn't mess with that.

My son has been 90% or higher for weight since he was about 9 months old. He's probably in the 80th% for height. At his 2 year appointment they said we could switch to lower fat milk since he didn't need the fat. As he has gotten older (he's almost 4.5 yo), he has definitely slimmed down, but he is still really heavy. I think he must have heavy bones or something because, other than his belly, he isn't a chunky monkey anymore.

Agree totally with @kgbee.

My son is 90%+ in weight and has been since birth. His height has been anywhere from 50% to 90%. He'll be three in September and still drinks whole milk. The pediatrician never suggested I switch. 90% sounds big but it totally depends on body shape and size. He's never been a "big chubby" kid. He is solid.

Go with your instinct!

If he's just drinking 6-8 oz a day, I would probably take a look at his overall diet--how many snacks per day, what his meals are made up of, how many processed foods I'm giving vs. how many whole foods, etc. Then I would probably make tweaks to the overall picture.
