Thank you, ladies for all your insights and recommendations. I have compiled a list of questions to discuss with my RE.

My RE practice does unmonitored IUIs when they are unmedicated. I think they start monitoring as soon as drugs are added, though. ETA: The practice is the best in my city with a great reputation - I had LH testing on CD3, then the day after my + OPK, and then 7 days later along with with progesterone. Since all my tests and scans came back clear and I had "proven ovulation," they recommended starting with an unmonitored IUI.

@doxielove: I would definitely want to be monitored there are other factors not just ovulation they an check for like uterine lining and number of follicles size etc

@doxielove: I would, also, add that you should call around to other clinics to see what their protocols are. From my own 3 year journey with IF, I would not feel comfortable going with a clinic that does not do any monitoring. Especially on fertility meds like clomid. Multiples are a risk on fertility meds.

I've had two successful pregnancies with IUI + femara. It was through Kaiser and I was not monitored. The RE provided the option to monitor, but suggested no monitoring for the first couple of cycles. Yes, you have more information, but it doesn't really mean that anything changes with the IUI/medication process, so why the extra cost and hassle? After two or three unsuccessful cycles, then yes, monitoring would be useful and could help address whether there's a need to change to a different fertility drug.

@doxielove: Actually, my memory failed me. Sorry. I did have monitoring but it was only one ultrasound to see the number of follicles. It wasn't daily or every other day monitoring. I also only did blood work every 6 months. Not every month.

I didn't really question them since it was all new to me.

same as @AGOLD

I did couple rounds of clomid and femara and was not monitored. The possibility of IUI was available depending on how I responded to medication. I did get one ultrasound for one of them to confirm ovulation but nothing as intense as I did when I finally switched to a RE clinic. I did injectibles and was heavily monitored (every other day, and then daily towards the end) before getting the IUI. I'm now 31 weeks :)

fingers crossed for you!

Mine were monitored. It seems weird not to- why not have the extra information to know how you are responding? It seems like a waste of time. We actually got pregnant on a monitored clomid and trigger TI cycle instead of an IUI cycle.

@doxielove: That is very interesting. I want to know why too! I've seen three different RE's and they have all done/will do monitoring, though my first one didn't do enough. I haven't had a cycle yet with my third one but as this isn't my first time I know what I should expect.

@KT326: Yes, I'm seeing an RE. I'm planning on grilling her on why she does things the way she does. No monitoring makes no sense to me, so maybe she'll shed some light on the matter.

I'm always wary of doctors who don't monitor medicated cycles. Are you seeing an RE? I would be afraid of producing multiple follicles. IUI is only slightly more effective than timed intercourse.

For us, IUI worked but I was monitored and on injectible meds (timed intercourse was too stressful for us).

I did clomid for three months and wasn't monitored. So I can imagine that just tacking an IUI on at the end wouldn't be much different than what I did. But when I did finally move on to IUIs, I did only injectible meds for those. Best of luck as you move forward with this!

My first clinic did unmonitored IUIs. I left that clinic after 3 failed IUIs. (They also didn't do any further testing to diagnose me, instead they gave me the 'unknown' diagnosis.) In the IF world, monitored is always better than unmonitored cycles. Plus, wouldn't you want to know if you had more than one follicle ready? You'll have to prepare yourself for the risk of multiples.

Has anyone ever heard of an IUI NOT being monitored...? My doctor does NOT do any ultrasounds during clomid + IUI cycles. She tells her patients to call when they get a + opk. When I inquired about any monitoring, I was told that this is how she handles all of her patients who need IUI but have proven ovulation on their own. I don't know much about IUI, so I'm not sure this is standard. I thought I'd have multiple ultrasounds to check on my follicles!!??

The plus side is 1 IUI would only be $380. If sacrificing monitoring means a lower cost, I'm not 100% sure I'm on board with this process...

fyi, I have a consultation to discuss further on the 29th.

Thoughts? Advice for an IUI newbie??
