I have to agree with all the above posters. I highly doubt the clinic didn't test for FSH and AMH. These tests are standard protocol for every patient now a days.

I’ll echo the others and say that I don’t believe that there is a test for egg “quality” - quantity/ ovarian reserves, yes. I’m sorry for your friend - I know it’s overwhelming.

i know for a fact that there's no test for egg quality. all of our tests came back with no problems (me and husband). the dr was wondering if i had poor egg quality (turns out i didn't), but she said she wouldn't know until doing IVF and retrieving eggs, because there is no test for it.

Wow, I can't believe they didn't do any AMH testing. It typically goes hand in hand with the FSH test. Unfortunately, those tests only really tell her what her egg reserve might be and not really egg quality. From what I know, I don't even think there is such a test (egg quality). They can only really tell that through trial and error and how fertilization rates are, how they mature, etc.

I guess I would recommend a few things. If she hasn't already, I'd tell her to start on Ubiquonol to improve egg quality for the off chance that she decides to do another cycle. It doesn't hurt to have her hubby start it as well to improve their chances. She also might want to have her DHEA tested, if she hasn't already, as if that is off, it can effect egg quality. It's an easy vitamin to take, but only if you really need it.

Have her look into some podcast episodes on Beat Infertility. I know that there are some on egg quality and it might give her ideas of questions to ask.

This totally sucks for her. I wonder if she stimmed too fast and/or has PCOS. That is a high egg retrieval number with low quality results and I've heard of that being common with those with PCOS. If she stimmed too quickly, the eggs wouldn't have been mature enough to form to their best possible and could lead to lower results. Just a few ideas.

Do you know what her FSH number is (you said that she does have that)?

When I did fertility treatments FSH was a regular part of the blood work, so I would be shocked if they weren’t testing it, less so if it’s just that she doesn’t know what it is. I didn’t have my AMH tested with the first two doctors either (4 IUIs, 1 IVF) but this was five years ago and I think they were just starting to do AMH testing. I do find it a bit surprising that it wouldn’t have been done (unless it was done and they just didn’t tell her the results) but I don’t think I’d say unethical. Given how many eggs she had her AMH probably would have been fine anyway.

If indeed they never did the day 3 tests I think that would be highly unusual and I would find a different clinic.

@SweetiePie: I asked her and she said...

“I had 21 eggs. 16 were mature and all 16 fertilized to embryos. Only 3 of the 16 made it through the first week. 2 are rated poorly and there’s only one good one but the 2 poorly and 1 good we’re all sent to the lab for genetic testing to see if they were ok to use or had any genetic disorders.”

@CatchAFallingStar: they didn’t test for them or she just doesn’t know if they tested/what the results were? My first go round I didn’t even know what AMH or FSH were so I couldn’t have told you if it was tested or what the results were.

And when you say only 1/16 is good, what does that mean? Only one was mature? Only 1 fertilized? Only 1 embryo made it to day 5? Or do you mean she only had one good embryo and she had them PGD tested?

Just trying to get a better picture.

One thing I’ve learned is that reproductive endocrinology is not as cut and dry as some of the other disciplines of medicine. My Clinic is one of the best in the country and my case the 2nd time still threw them for many loops. It’s a lot of trial and error and test scores don’t always mean exactly what we would interpret them to mean (think of how many women just here on HB have been told that their AMH was too low to conceive and then they went on to get pregnant naturally).

Unless they flat out did not test for those things (which I have a hard time believing unless it’s a really low rated clinic) I wouldn’t jump to the conclusion that they did something wrong.
If they didn’t test for those though, I’d be finding a new clinic immediately. That would be unacceptable.

Ummm...what?

The first thing my fertility doctor pushed was full bloodwork and semen analysis. We decided to forgo testing for one cycle since we've conceived three times and then to do it, but it was the very first thing mentioned (and we are just doing monitoring, femara, and trigger.)

I am so sad for your friend.

Thankfully, I have only had to have IUIs and have never experienced IVF. But, I feel like I know quite a bit about it just because I've dealt with fertility issues and read/listen to A LOT pertaining the process.

A friend of mine is going through IVF right now for the first time after years of no success with IUI or TI. After spending $30,000, they just found out that out of 16 eggs, only one is viable and that her egg quality is poor. (She's only in her early 30's.)

She told me they didn't do any testing in her egg quality prior to recommending and pursuing IVF. After years of treatment with the same clinic, she doesn't know her FSH level and hasn't even had her AMH checked (which I know is more for quantity than quality, but STILL!).

My question is, is her clinic being unethical by not doing these tests and taking her $30,000 and all her hopes and dreams for a baby and pursuing IVF anyway?
