It's supposed to be something that you are given when they give you the diaper bag. If you are part of another Kaiser, make sure to ask! I was discharged 8 weeks ago but am going to go in tomorrow to get mine. They didn't even ask for my MR#.</p> <p>Hope this helps somebody! </p>
