Just to chime in with a different perspective, I did not like my hospital employed CNM at all. Honestly she had awful bedside manner, couldn't seem to handle me bringing a doula, and determined I needed a c section. It was the OB that was called in to do the c section that gave me the OK to deliver vaginally. I still like the stats and the idea of a CNM but like all things in life individual experiences will vary.

Another person chiming in to say I loved my CNM. She started her career as a L&D nurse and then went back to school for her msn. She was amazing, and I'd absolutely do it again. It was experience and compassion which I didn't feel with the ob I saw occasionally.

Another here who delivered with a midwife group at a hospital. I will 100% do the same (with the same midwives) if I am able to next time.
My delivering midwife was not my primary, but they rotated appointments in the last couple months of pregnancy to make sure you had met each one and they got to know you and your pregnancy. She was there almost the entire time during my labor and really listened to me and I am certain her encouragement was essential to my success in having the birth I hoped for.
The midwife group was part of a larger practice, 4 of them and 8 OBs. They do an open house with most of the docs/midwives who are not on call once a month so that you can attend before your due date and meet those you haven't had the chance to.
Oh and BIG congrats on #2 :) I can't believe how big these babies of ours have gotten!

Add me to the list of people who love midwives in a hospital setting. With my first LO, I saw an OB/midwife group. An OB attended my particular delivery, but I loved the care I got from the midwife during my pregnancy and hospital stay. This time I'm seeing a midwife group that is part of the hospital. You can choose to use their birth center or the hospital. I've felt very supported and listened to throughout my care. I'm anticipating that being the same through my delivery, too.

I used the CNMs at my hospital for my first birth and liked them - in the end I did end up with an emergency c-section, but they spent many many hours trying to prevent it. Obviously an OB stepped in at that point and it wasn't someone I had met, but we weren't exactly making small talk during surgery and it didn't really matter. This time I'm using an OB because I'm at a different hospital and for whatever reason I just didn't really click with their midwives, and the OBs I've been seeing there have been very supportive. But exceptions like that aside, I agree that using a CNM at a hospital is pretty much the best of both worlds.

Arden answered with much more info than I could, but my personal experience what just what you described. Midwife practice in hospital. I loved it and will definitely do the same next time. Because of a few factors during the birth- meconium, heart rate issues- they did have an OB come in NF sdidtwiththe delivery at the end, but it hardly mattered to me since the same midwife and nurse stayed with me anyway, they just weren't the ones actually down there. I had a vacuum assist which I think only the OB would do, not a midwife, but I could absolutely be wrong about that.

I think it's the perfect compromise for me!

Arden already replied but just thought I'd reply. CNM is the actual certification while MSN is the amount of education. It's law now that a CNM has to get a DNP, unless you were "grandfathered" in. So some CNM's may have a DNP instead, which is a doctorate degree.

I personally liked the midwife model of care, and it felt more personal, like having a great relationship with the person who would deliver my baby. When I ended up needing to be medically transferred to the hospital, she came with (and the practice was just a mile from the local hospital, and only about 10 miles from the hospital they had a partnership with), and while she was with me the majority of the time- I saw the delivering OB for all of 10 minutes? I really appreciated that aspect of it.

Good luck with your decision!

@coopsmama: no, that is great! Arden answered my specific questions. It is a group, as well, which I don't think I said. The other midwife groups I'd looked at all delivered out of hospital and didn't allow epidurals - neither of which I was interested in. I'm excited! We're moving to a larger area, and I will suddenly have many more options.

I'm using a CNM for this pregnancy and am loving it. It's actually a group of 4 CNM's, so I'm not quite sure who will be at the delivery (depends who is on call), but I love them all. I used a traditional out-of-hospital midwife last pregnancy and had to be transported to the hospital for maternal exhaustion - nothing life threatening. But after that experience, I really wanted to still use a care provider who would have that midwife "feel" while also allowing for a hospital birth and this has been a great compromise. I have also really felt well supported in the underlying emotions surrounding birth and in discussing options for labor such as epidural, natural birth, etc. I would highly recommend it if those CNM's are anything like mine!

That's not really addressing your questions above, I realize, but I just thought I'd chime in with my experience so far. If you have any specific questions I'd be happy to answer them.

@Arden: thank you so much! That was the only thing I was thinking of, that they couldn't operate. My OB was out of town at my first birth, so the switcheroo wouldn't bug me.

It just seems like the best of both worlds - a midwife who delivers in a hospital, just in case ... I'm trying to think of the "catch" ha!!

Reasons to use a midwife: National research shows that midwives achieve the same outcomes as physicians without disrupting the natural birth process as often. Research also shows that midwives have reduced cesarean section rates by more than 50% with no compromise in safety. In fact, care from midwives has significantly lower rates of induced labor, episiotomies, and forcep births. Midwives are most actively involved in a woman's preparation for childbirth. That kind of support reduces the length of labor, avoids unnecessary interventions and improves birth outcomes.

Reasons not to use one: if your pregnancy becomes high risk at any point, you would have to transfer care to a OB. This would mean you could end up with an unfamiliar provider attending your birth. Sometimes midwives share a practice with an OB so you meet the OB during pregnancy just so that you can be more comfortable with him/her if you end up transferring care.

CNM MSN stands for Certified Nurse Midwife, Masters of Science in Nursing. They can do basically everything except surgery. They can order epidurals, prescriptions for any drugs, etc.

They can do everything involved in birth, but if you end up needing a c-section that would be performed by a OB, with the midwife usually still attending.

They have 6-8 years of schooling, basically Nurse Practioners with an additional midwifery speciality certification.

Certified nurse-midwives are regulated at 2 different levels. Licensing occurs at the state level and falls under specific state laws. As with other advanced practice nurses, license requirements for CNMs can vary from state to state.

Certification is done through a national organization and all states have the same requirements for professional practice standards. Only graduates of nurse-midwifery programs accredited by the American College of Nurse-Midwives (ACNM) are eligible to take the certification exam.

Google is not helping. What exactly is different between an OB and a midwife (they say CNM MSN, which I'm assuming are some sort of certification)?

I found a midwife group that was recommended, they deliver at the hospital, and allow epidurals. Assuming I am low risk; are there any compelling reasons to not use them? To use them?
