I also used a sterilized needle and worked at the edges until I was able to massage the milk to release. It helped to either take a hot shower prior or soak your nipple with some olive oil to soften the skin, then massage the area to make the bleb protrude before working on it. It hurt to do but, like PP's, the relief was immediate once milk started spraying out. Hope you feel better soon!

I used a sterilized needle too. If it's really bad, a hot shower helps before working on it. Squeeze the area around the bleb when you're trying to work it out, so it starts to stick out like a pencil eraser.

I've dealt with a couple. It's so gross but yep, sterilize very sharp tweezers. Remember that you aren't "popping" the bleb like a zit or blister. Rather you are sort of lifting off the layer of skin that is trapping the milk. Then afterwards apply triple ointment (it's at any grocery/drug store) to prevent infection. And do an epsom soak (1 part epsom salt to 2 parts warm water, hold over nipple for two minutes several times a day). The good news is I felt better almost immediately.

Oh gosh, it's been awhile since I've had one (last summer 2013) but I ecentually just had to sterilize tweezers and pull the skin off. It hurt SO bad but felt much better immediately after.
