I accidentally reintroduced dairy to my 5 month old when I was eating honey almonds that I didn't realize had butter as an ingredient. After I realized it, and she seemed fine, I started with the dairy ladder and tried muffins baked with Greek yogurt. I had half a muffin each day for 3 days and it became clear she wasn't ready for dairy yet. No blood in her poop again yet, but they got greenish and mucousy, she is spitting up much more than usual, has an eczema-like rash on her torso and back, and is very fussy. It probably took 48 hours for me to determine she was reacting. I don't think I'll try again until at least 9 months.

@pwnstar: @juli7: @catlady: @kes18: @petitenoisette: @Pollywog: @MrsADS: @Mrs. Microscope: You guys, thank you so much! This was all really great information and so much first hand experience!! I really, really appreciate it! I definitely think I will hold off for now and just wait to do anything until we get a bit of a handle on solids. The dairy ladder concept is really interesting... I think this will be helpful! Of course I am hopeful that she will just outgrow it because I definitely do not want my super-sad baby back!!

We reintroduced at 10.5 months because I wasn't pumping enough and my son wouldn't drink any elemental formulas. He was fine with dairy at that point, and I think we introduced soy a few weeks later. I was nervous that I wouldn't notice any blood in his stool (vs in the newborn poop where it was very easy to see the blood), but my pediatrician assured me that if he still had MSPI, it would be obvious enough that I would know (either seeing blood or excessively gassy, spitting up, etc). We gave him a few small bites of yogurt or cheese every day for a couple days, and then gradually increased what we gave him.

I've done this twice, although I don't have too much to add. With LO1, she had definite MSPI and possible other issues that led to her testing positive for blood even after I had eliminated dairy, soy, and a ton of other things for months. Because we never got her to test negative, our pediatrician didn't want me to re-introduce anything into my diet or hers until she was at least a year. So for her, I just sucked it up and we gradually re-introduced everything between 12 and 14 months (took me awhile because I had eliminated so much). At that stage, she didn't seem to have any reactions to anything, and she had also tested negative for allergies. I don't know how she would have reacted earlier though.

With LO2, I gave up dairy/soy while pregnant in case he had MSPI too. After 2 months, I slowly re-introduced them back into my diet (for dairy, I used the order that kes18 mentioned) and just watched for signs of MSPI. He seemed fine, so eventually I just brought everything back in. He's 6 months old now and still doing fine, so it seems like he probably never had it. He did have reflux as a younger baby and still doesn't sleep well, but I have learned not to use those as signs of MSPI.

@GreenThumb:
They usually recommend you wait until 9-12mo to start but it's not unheard of to try earlier depending on symptoms. My ped also told me I could try at 6mo. You can try the dairy ladder, just google it. Lowest (lowest amount of proteins) is baked in dairy which is dairy baked at 350 (so muffins, cake, cookie). By 9mo it would probably be best just to give to baby directly rather than yourself. And if baby passes then you can eat it. I'm am trialing in a little bit right now with my 6.5 mo old but (as far as we know) he did not have blood in his poop- just colicky, gassy, and he vomited up a lot due to reflux (soothing his tummy by nursing until he puked it all back up). I have just been eating some banana bread with 1/4 c yogurt in it (baked in dairy). Just wanted to see if he was okay with baked in dairy. Today is my 3rd day. He isn't colicky but he's spit up a few times (def not vomit though!) and has had trouble going to sleep but I'm not sure whether it's the dairy or not. He's a shitty sleeper anyway we'll see. Maybe I'll stop for a few weeks to see if he improves or not. Some babies you can eat items up to a certain point on the ladder before they react. Rule is to do 1 step for 3 days and move on if no symptoms. You can find other versions, should be pretty similar. So you should see symptoms return within 3 days if baby is reacting
1- baked in dairy
2- normal butter
3- yogurt
4- cooked cheese
5- uncooked cheese
6- heated cows milk (so like pudding)
7- cow's milk (fresh)

I found it so hard to figure out if my LO was reacting to stuff from my diet when I tried to reintroduce. It was like is she fussing bc of the dairy or is she teething? I ended up waiting until she was almost 18 months old to reintroduce it and then it was fine and she could have it too. I'm sure she was fine before then but it was just easier for me to keep with the diet (and I also had eliminated many other foods initially so when I got it down to just eliminating soy/dairy it didn't seem that bad anymore).

I agree with others and especially because you said she's still a pretty fussy baby to wait a little longer.

I found symptoms are between 6-18 hours after he nurses and it takes an hour to get into my milk. He will start puking then have slimy diapers.

I was able to have hidden soy at 12 months and soy sauce at 13 months. I was able to have baked dairy and baked cheese in moderation at 15 months. He still can't tolerate yogurt or unbaked dairy.

Good luck!

I agree I would wait to trial, although we followed the advice of my son's pefiatric GI. He had very severe reflux and vomiting in addition to the trypical mspi poops. So, it was easy for me to tell if something I ate bothered him. He would scream nonstop, not sleep, and start vomiting a million times. I trialed dairy in my diet at about 15 months because of the severity of his reactions. And he passed!

@skipra: Totally agree. It's much easier to get a read on how he's doing with new foods because of the formula (it's constant--unlike what my diet would be if I was still BFing).

@GreenThumb: my DD was mspi from 5 weeks, and we did a trial at just after 3 months old as per pediatrician recommendation, and she passed. So it isn't unheard of. She also has bloody diapers, with mucous and green as her primary mspi symptom, also with facial rash. She had no problems until we tried to introduce whole milk at 12 months, and the symptoms came back. Easy fix to not give her whole milk, and she can eat cheese, yogurt, ice cream, baked in milk, etc. just fine. All kids are different.

@GreenThumb: It was my Ped GI who helped me realize that it was okay to wait. It must have been the expression that crossed my face when he talked about when I could try to reintroduce dairy/soy (starting with soy), and he told me that I didn't have to do it if I wasn't ready/didn't want to risk it.

@pwnstar: yes, it's different introducing into breast milk vs. directly with food. I feel like I gave it to my oldest too early and now he has so many feeding issues. I was much more aware and conservative introducing it with my second and he has zero issues with any food now. My youngest is almost 9 months and seems to be the most sensitive so I have not had more than an occasional bite here and there. But definitely no direct soy or dairy until a year.

@pwnstar: I appreciate you letting me know... my pediatrician gave me very little info and was pretty nonchalant about the whole thing, so I thought trying to reintroduce at 6 months was pretty standard. I don't want to mess with what is working either, and I guess I don't have to if I don't want to!

@GreenThumb: MY son is MSPI. I'm not going to introduce dairy/soy again until at least 12 months (he just turned 10 mos.). After doing an elimination diet and ultimately switching to elemental formula, now that he's doing so much better, I'm hella reluctant to mess with it!

@skipra: Thank you! This is helpful. It does seem difficult to distinguish if there is still a problem right when we are just starting solids. And clearly I'm a bit nervous about making her so sad and uncomfortable again. Waiting until 9 months might be a good choice since I'm not really having trouble with my diet as is.

It is harder to tell once they start solids because the poop is so different anyway and you don't necessarily know if it's something you ate or something she ate. My suggestion is to only give her foods that you know she is ok with while you reintroduce to your diet and really pay attention to the other symptoms besides bloody poop. Is she getting progressively fussier and not sleeping as well? Is she pooping more frequently? Also try soy first because it sticks around in your milk and her body for less time. I have found that if they are not ok with it yet, mamas intuition will let you know within a couple days.

Eta: as a mom of 3 mspi babies, I think it's best to wait until 9 months, once their tummies are more accustomed to a variety of different solids.

@Mrs. Microscope: Well, clearly! I guess maybe that was a stupid question. But what I am concerned about is how quickly this might happen. I don't want to get her to the point of having blood in her diapers and like I said, she is generally pretty fussy anyhow. So I'm not sure how quickly I will I might see the impact of the dairy. I'm concerned about her being uncomfortable.

My EBF DD2 is suspected to have MSPI so I eliminated dairy and soy at about 3.5 months. I was told by my ped that we could try reintroducing it into my diet at about 6-7 months as a trial and to see how she did, but was given little information about what to expect if there was still an issue. She is generally a pretty fussy/gassy baby anyhow, so while she is definitely better with the elimination, I'm not sure how I will tell if she has outgrown the sensitivity or not. Do any MSPI moms have any advice? We only discovered the problem when there was blood in her diapers. I'd really hope to not get to that point again. TX!
