@SeptMomma12: we haven't had any puke episodes since Wednesday. (She has spit up daily, but small, much more normal amounts). Fingers crossed that we are over the worst. I have been nursing "uphill", stopping to burp every few minutes when she's gulping, and usually only offering one side. When she's finished in one side, I take her off and give her 5-10 min before deciding whether to offer the other side. If she's rooting, I offer, but usually she falls asleep. I did get mastitis on Thursdsy, which has reduced my supply. I think that's actually been a good thing as far as her vomiting is concerned.

@78h2o: I think it's very possible that your Lo is still learning how to regulate her sucking and feeding and the puking was a result of gorging herself! Mother Nature designed new mothers to have abundant / oversupply so that a newborn would still get plenty of milk while learning to feed so great supply combined with a well-feeding newbie can be a recipe for copious amounts of spew.

I don't think there's any harm in offering a paci if she's feeding well and frequently, it could be that she would rather suck, I also wouldn't be too concerned about the spew if LO is otherwise happy and content.

Something you could also try is spilling 1-2 ounces of water on your bench, it can look like a lot more liquid than it actually is!

@Maysprout: That's good to hear. I really feel like the issue might be a forceful letdown. I can't feel letdown, so I don't really know when it's happening, but it seems like right after she first latches, she is gulping, gulping, gulping... almost like she's funneling milk! She will also occasionally pull off and cough a bit, like it was too much or went down the wrong way. I cut back on how much I was feeding her today and also tried to nurse "uphill" (to reduce milk flow) and she didn't puke, so we'll see...

My older daughter puked and it came out her nose several times in the newborn stage. I tried to slow down her feeds just slightly, like right when my milk would let down Id remove her for a second, sometimes I'd even use my manual pump just to take a little of the pressure off. But our pediatrician was unconcerned and said it happens with some babies. It didn't last too long.

@Mrs.KMM: she didn't puke after the 3.3 oz feeding, that is just the only weighed feed I have to go on. I did think that was a lot for only 4 days old... esp because I would say it seemed like a small-med feed compared to other feeds where she'd nursed for longer or from both sides.

I talked to two friends yesterday whose LOs had reflux...it sounds like their LOs were fussy a lot, whereas my LO has not really been fussy at all so far. It also sounded like their LOs appeared to be in pain, which I haven't really seen with mine.

@marionberry: thanks! Glad to hear your ped wasn't concerned. I am going to talk to ours at her appointment on Friday.

@anbanan15: I hope it's not FPIES...I remember reading about it on here awhile back (maybe something you posted?). It sounds like a challenging condition to deal with. Thanks for mentioning it though. It's definitely something we need to rule out. ETA: I'm not sure if it is projectile or not. my sister said she thought it was. The last time she did it, she was in the Ergo, so it was hard to tell. I think she usually does it within about 30 min of eating, but I'll try to pay closer attention.

I had this very same issue with one of our twins. He bottle feeds and was taking in a lot for a baby his size, right from the beginning. He'd drink a lot, be perfectly happy, and then anywhere from minutes to hours later, projectile vomit huge amounts from his nose and mouth. His brother drank the same amount and while he was a happy spitter, never projectile vomited. I talked to our ped about it and he didn't sound concerned at all, in fact he didn't tell me to do anything different. As he's gotten a bit older it has decreased, although it still happens on occasion. I have just sucked it up as normal for us, and gotten used to the occasional explosion. Both twins are in the 75th and 50th percentiles for weight respectively, so I think they're doing just fine. I am glad to hear that this happens to other people though!

LO is 11 days old now and it isn't uncommon for her to eat 2.5-3 oz per feeding (and we have even had a 4oz feeding!) and she doesn't throw up. So I have a hard time thinking you're over feeding. I don't know much about how reflux presents in babies though.

My first thought was FPIES, however it sounds like she is vomiting immediately following a feed. With FPIES the reaction (projectile vomiting) is typically delayed 1-3 hrs after a feed with 2 hrs after being the most common). Is it projectile or just larger quantities? Also our docs always said that baby vomit can look like a lot more than it is because it is thin and spreads.

My DS had FPIES and also really bad reflux. He nursed non-stop and projectile vomited all the time until we figured out what it what and what foods in my diet were causing the issues. I just wanted to mention it, but again based on what you described it doesn't necessarily sound like it.

I would contact the pedi and LC again, but if there is no medical reason for the vomiting it might be something you just have to work through until your supply meets her demand.

My DD did a lot of puking through her nose in the early days and it did seem like a lot of milk every time. If your DD isn't fussy, I would guess its over eating and not reflux. Both of my LOs puked a lot, DD more than DS, and an LC told me to offer a paci right after feeds for like 5 minutes, it worked really well with my son to help keep milk down. My DD hated the paci though, so it didn't work as well with her to help curb puking.

@78h2o: I don't know if it's overfeeding, but mine does the same thing because I have an oversupply and quick letdown. If your LO is coughing while eating and taking in a lot quickly, it could be a quick letdown issue. If I feed mine lying flat on my back so she has to actively suck the milk upwards, she doesn't barf afterwards. If I don't, she has a really big puke - like actual puke where she retches for a bit first and then actually barfs, not just a spit-up. Oh, and she has to be burped frequently too - that helps!

@dc yoga bee: yeah and I've been sitting here thinking about it and I think you're right they can. My daughter will eat until she throws up but if I give her the paci she chills out until she needs to eat. So yea I think they can, I've been told the same thing though and I know with my son I didn't have oversupply and he'd eat and eat and I'd swear he was full but a paci would never satisfy him and I just think it was the difference in supply

@dc yoga bee: I think over feeding can happen with breastfeeding too. I think it was my body's fault tho. I know each of my breasts hold a lot of milk and coupled with the fact that I have a forceful letdown, my DD would get too much milk. The spit up and throwing up of milk didn't stop until she was older and could handle the amount of milk/forceful letdown.

My DD would nurse too much and have projectile spit up. It looked like all the milk she just drank came back up in this huge stream (and it seemed i was usually in it's path, milk all over me lol). But she never cried or seemed bothered by it so I just chalked it up to nursing too much/ just normal baby spit up. It would happen a couple times a week when she was a newborn. Still happened occasionally from 3-6 months. I know my boobs hold a lot of milk tho and I have a forceful letdown. She would always pop off when my letdown would happen and I would have to catch the milk in towel. After she would have these huge spit ups, I would try to wait 1-2 hours before feeding her again.

I would continue to nurse on demand. Can you just offer one side and not both breast for each feeding? My DD always only took one breast per feeding. And after she nurses, let her have the pacifier if you think she just wants it for comfort.

For some babies it's a total myth that they won't overeat. My LO would eat as much breast milk as I would give her and never knew when to stop. It's definitely sounds like your LO is eating more than her stomach can hold.

I also agree it's probably overfeeding my 3 week old is lucky to take 2 oz every 2 hours from a bottle. I nurse her in demand and she spits up though not nearly that amount. I use a pacifier she seems to need it, my first LO used me as a paci and I don't want that to happen this time. She lets me know if the paci isn't enough ;)

@dc yoga bee: if nursing on demand yes but a bottle was involved. You can definitely over feed with a bottle. :)

@78h2o: I would agree on the over feeding. When my son was 9 days old he was probably taking 1.5-2 oz of formula and a lot of people thought THAT was a lot.
I asked my ped and she said "if you over feed him you'll know. He will vomit as opposed to spit up". He only truly vomited once and it happened to be a bigger than usual feeding.

I know that pacifiers can be controversial, by if she nurses fine, if say trust your instinct and maybe try that, or give her her hands or a finger to suck on? I agree that it's probably not reflux. If you start to see signs on discomfort, especially arching back, then it might be.

I have a similar problem, but the lactation consultant, ped, nurses, and everything I read says you CAN't Overfeed a breastfed baby, to keep on nursing on demand, and not to give her the pacifier. "Baby is sending sinals to establish your supply so continue to giver breast whenever she asks," is what the lactation consultant told me yesterday. I'm at a loss too. She eats and eats and eats, throws up, and wants to eat again. I'm scared to give her the pacifier though, and my finger does not console her in the least. I don't see how they're not overeating!

I am going to call her doctor tomorrow, but I really feel likes she's eating too much. It's always happened after she's eaten what seemed like a lot.

At 4 days old, she shouldn't really have been eating 3.3oz from one side (esp considering she usually nurses from both sides).

I think it might be over feeding. My LO never was a spitter, but one time when she was about 5 weeks old she kept acting like she wanted more and more, so we kept giving it.....and then she had an adult sized throw up---all over DH---it ran from his shoulder all the way down his body to the floor, it was so much.

Wow that's a big feeding for a 9 day old. What if you cut the session short but feed her more often? That may help if it's reflux. And I know they all say no pacifier, but I let T have one sometimes because I don't want to be his pacifier! You can just stop giving it if you sense it's causing problems

My breastfed baby is 9 days old and sp far has majorly puked about 5x. Not like a little bit of spit up - major puking that comes out her nose and mouth at the same time. She did it yesterday while getting neeborn photos, and both the photographer and my sister (who have kids if their own) said they'd never seen anything like it. My sister said it was an "adult-sized" amount of puke and that's probably accurate. Maybe 6-8 oz??? She seems fine afterwards and immediately starts rooting again. I feel like maybe she isn't refulated how much she's eatiing? When she was 4 days old, I took her to a breastfeeding support group and she ate 3.2 oz in about about 20 min. That time she just nursed in one side, but often she nurses fri both. It seemed like that amount was more than other babies her age were consing during the group.

The lactation consultant told me no pacifier until 4 weeks, but I'm worried LO might be nursing for comfort and not understanding how to suck without taking in too much. Should I not feed on demand? Use a pacifier when she's rooting? She will suck on her hands, but she prefers the pacifier.

If it's not overfeeding, I'm thinking it's reflux? She occasionally will cough/choke a little bit, like milk when down the wrong way or something came up.

Anyone have experience with this?

ETA: before she puked yesterday, she nursed for 20 min at 8am, took 3oz of BM via bottle (because I was wearing a non-nursing friendly dress for photos) at 10am and the nursed on both sides at 11:30. I only fed her each time because she was fussing and rooting. She puked around 11:45-12.
