Today's appointment went really well and I feel much more comfortable moving forward. I have a lap scheduled for May 22 :) During the pelvic exam he took a sample of cm...and it looks like I am ovulating today. This may be TMI...but I got to see the CM under the microscope, when it is ovulation time, it "ferns" when it dries and it looks super cool. I could also see DH's swimmers from last night's deposit. Maybe that would be a bit much for some people, but I thought it was super cool.

@doxielove: exactly...its not about what is the best for me its about what insurance will covrr.

@mrswin: im in a decent amount of pain and have very painful periods...Combine that with the physical exam and that is her best guess. Unfortunately, they were at the fertility clinic 4 hours away at that point because they had been trying for years.

@Jenn23: the hsg would be about $650...and I think we will end up going that route....I was just so frustrated that I wasnt being heard yesterday. I will be switching drs though....and most likely it will be closer which will be a good thing!

It will all work out! Thank you all for ur kind words and support.

@owlteach: Aw, big hugs to you!! I'm so sorry you had such a crappy day. I know how awful it is dealing with insurance and doctor's and feeling like you aren't getting anywhere at times. It should be so much easier! I agree with you to check your tubes just to be sure they are clear. Maybe you should do the surgery then since it is covered? How much will the HSG be out of pocket?

@owlteach: I have endo that was diagnosed over a year ago though a lap. Why do they suspect endo? Laps generally have not been shown to significantly improve fertility so if it was me I would want to do the HSG and a SA first before anything else. There is no reason to move onto a lap unless you are dealing with severe pain and are looking to manage that.

I would get the codes you need and then call the insurance company directly, maybe it will be covered as diagnostic.

In terms of finding another doctor, is it possible to use the same person your co-worker did?

@owlteach: Ugh, I'm sorry to hear your update. I agree, our medical system is pretty jacked. 9 times out of 10, I find myself making health decisions based off of what my insurance covers (and not necessarily what's the best choice for me). I'm glad you are deciding to part ways with your current doctor, though!

@doxielove: @LindsayInNY: Thank you.

All I have done with this dr. is play telephone. Today they told me that any dr's office that gets HSG's covered without infertility coverage is committing insurance fraud.

Honestly, my mom offered to pay for half the HSG.....but I pretty much told my dr's office that it was probably time to go our separate ways. Of course, then I have had zero luck getting anyone to answer my questions over the phone. Basically, I don't want to go through all the testing again...only to be in this exact same place. I called OHSU, which if 4 hours from me, but where my friend had an HSG that was paid for by our insurance. They told me I would need to book a $450 appointment that is not covered by insurance before they could even discuss what my insurance covers and that it is too bad that I don't live in Portland and that it would be such a hassle for me to travel there for that appointment.

Frustrated doesn't even begin to cover how I feel. I have never felt this helpless in my life. I don't know where to go, who to ask for help from, or what to do. Our medical system is so f***ed up.

I'm fine with the actual medical treatments, I just don't understand why I am being pushed to the most invasive procedure (laprascopy) when verifying that my tubes are open and then using clomid would be cheaper for everyone. I asked the nurse today what of my 3 treatments the dr. recommends and she said that she recommends the laprascopy because it is what my insurance covers...but that she thinks the HSG and clomid has just as good of chance at actually working.

@owlteach: I'm so sorry hun. I oftentimes feel like it's not fair I have to go through all this work just to get pregnant - it truly is exhausting! My insurance doesn't cover anything RE related so I'm stuck working with my OB for now (who is great - but he's got pregnant women to worry about, too, so sometimes I feel like I'm being put on the backburner by his office...)

@owlteach: ((Hugs)) I'm sorry... I feel your frustration when it comes to doctor's offices and insurance. I called mine the other day, spoke to the nurse who spoke to my doctor and called me back - it felt like a bad game of telephone and I felt like most of my message didn't even make it to my doctor. It's so frustrating.

This morning when I called and asked about a timeline with the clomid, they told me that they usually do 3-4 cycles on clomid and then move on to IUI's. I asked if they would do that without addressing the potential endo and mentioned that I was confused because we've tried the 2-3 months of regular cycles as planned, and I thought the lap to diagnose endo was supposed to be next. She then looked at my chart and said "Oh, yes it does look like the dr. suggested scheduling your surgery for April". I then asked the nurse to clarify that with the dr. and if she could find out how much I would end up paying for the hsg and the surgery.

So, I called back a couple of hours later and was told the dr. could do the hsg or the surgery and that the surgery would also allow her to check my tubes. The surgery will be covered by my insurance, but that the hsg would not be....so they recommend the surgery. I tried pushing and getting more info because another teacher with the same insurance was able to get their hsg covered at a fertility clinic....but they told me to call the insurance companies myself. Well, that was a waste of my time because they couldn't look it up without cpt codes. So, at 4:55 our time I called the dr. back to get the codes, and they'd already turned their voicemail on for the night.

I ended the evening sobbing at my desk because it is all just so much harder than it needs to be. I feel like I'm being forced into making a decision, not based on what could be best for me, but on what my insurance will cover. I don't feel like the dr's office is advocating for me, and I'm exhausted. Honestly, I am thinking about calling my secretary and asking for someone to cover me for a class period tomorrow so that I can call my Dr's office. My students have a big contest coming up and we will be going to a community meeting at lunch tomorrow to present and to another event right after school, so I will literally not have any time to myself otherwise. It's been a crazy week and trying to squeeze these conversations into 5 minute passing breaks is just not working.

Tomorrow is a new day, and it WILL be a better one :)

@CatchAFallingStar: They do suspect endometriosis, however she is trying to see if I can get pregnant inspite of it. I just don't see the point in trying clomid if my tubes are blocked...because it won't matter if my ovulation is "improved" if sperm can't meet egg.

@owlteach: Maybe they're prescribing Clomid to see if it helps with ovulation... As far as HSG, I think mine was about $800, but the doctor billed it as a non-fertility related procedure, so I didn't have to pay. If they suspect endometriosis, though, that can't be detected with an HSG.

Hi there, I think you shouldn't take clomid until you've check the SA results and had your tubes checked. I, unfortunately, am one of those women who has a blocked tube (other was removed in 2010) so after taking Clomid/IUI, we found out it was a complete waste of time and money. I developed a cyst right after the IUI, had surgery and that is when my RE saw that my tube was now blocked. Once we found out my tube was blocked, we had no choice but IVF. So be sure to check to make sure you don't have blockage, just in case. Also, I highly recommend seeing an RE and having monitoring done when taking Clomid. It is a scary thought to know you could potentially have lots of eggs and all could fertilize! Good luck. :)

@owlteach: hi! I have no fertility coverage, but "family planning." My RE's office was AWESOME about getting all the testing covered for me. Even Clomid and Letrozole! It wasn't until I moved onto injectables that it raised the red flag for fertility treatments.

I'm a strong believer in (1) doing all the testing first; and (2) being monitored during fertility treatments. Even though doctors seem willing to hand out Clomid like candy, it's a serious drugs w/potentially serious side effects.

Are you seeing your OB/GYN? Maybe it's time to move onto a RE! Good luck--I'm glad you stuck to your guns!

Maybe call your doctor's office back and ask for the benefits of using clomid before an hsg?

The way my doctor explained things to me, he thinks it's a waste using clomid if your tubes end of being blocked. (His advice may have been specific to me, though, since he suspects endometriosis based off my symptoms and family history). I'm getting my hsg done in May and my insurance will cover it as an IF test (my insurance covers all IF testing but NO IF treatments).

So, I finally got the results back from my 4th progesterone test...according to the first three I ovulated, but not this cycle. There was a non-tested cycle that my dr. doesn't that I ovulated during either. I was under the impression, that we were going to monitor and try for a few more months and then look at surgery for endometriosis.

However, today when my dr's office called they told me they were writing me a prescription for 50 mg of clomid, and for dh to get a sa. I understand the sa, and so does he. What I don't understand is the clomid. I'm not sure if I am comfortable taking it unmonitored (we would still test for progesterone) and I am concerned because what if my tubes are blocked?

She did say that she wanted to get the sa results back before they had me take the Clomid.

Should I ask for an hsg first? I am double covered under DH and I's insurance...but with no fertility coverage. Has anyone ever had experience with an hsg who doesn't have fertility coverage? Any idea if they are covered or not?

I can't believe she just had her nurse call and tell me that she was prescribing fertility drugs. I don't know, maybe I am making a mountain out of a mole hill. After I asked a million and a half questions, the nurse said she would either have the dr. call me back or she would try and get some more info and she would call me back. Well, I called back 5 hours later at 4:30 and happened to catch the dr. and was able to answer a few more questions.
