LO's pediatrician also works at the Newman clinic. Instead of paying for the APNO, he had me mix up a similar version at home.
It was a dab of polysporin and a dab of yeast infection cream mixed together. It healed everything very quickly.
He called APNO the big guns.

You are such a trooper, I hope things get easier soon!

I also had a similar experience & was sooo thankful to have my Ob prescribe an ointment for me. I was prescribed bactroban which is like APNO lite! You don't have to go to a compounding pharmacy for it & was far cheaper than APNO. Of course I would've paid thousands of millions of dollars at the time for some relief! You can always move into the APNO if bactroban doesn't work. I still use it every once in a while if we have a bad latch & it helps immediately. The other great thing is that you don't have to remember to wipe off before feeding like with some other treatments. I found lanolin to not be as effective for me as it made my nipples super itchy. Which was less than ideal!
My lo was a sleepy eater too so I had to strip him down which he hated, but worked!

I cannot recommend Apno enough, it is a lifesaver. As mentioned you need to find a compounding pharmacy that can make it for you but it is so helpful. And yes breast shells to protect your nips as they heal. Also try coconut oil to moisturizer. And I would start a probiotic to ward off thrush as sorrycharlie said. Hang in there! My nips were a disaster for the first two months and then they got back to normal.

I had trouble with infected open wounds that weren't healing too, I'm so sorry you're going through it!

here's what I did:

lanolin before feeding/pumping
when done feeding, wipe nips off (I had a super soft cloth that I disinfected regularly!) because I found that LOs saliva made nips really dry. of course, that doesn't matter if you're pumping :)
rub some breast milk on nips (be gentle, obvs ;) may or may not have made myself cry being too rough)
let that dry (I used a little paper fan to speed up the drying)
use more lanolin (I found that rubbing the lanolin between my fingers to soften it was necessary, else it was too gummy)
apply small square of cuticerin ( http://www.medline.com/jump/product/x/Z05-PF34881 ) to nips. this stopped my bra sticking to the nip wounds, and is also meant to help heal wounds too. I got mine from my midwife, but I bet you can get them at a pharmacy

my LC and midwife told me off for putting nips away soggy. even after showers you should be making sure they're dry!

just a thought, have you checked to make sure the nipple flanges on your pump are the right size? I used too big ones for a while and I swear it nearly sucked my nipple off! (okay, maybe a slight exaggeration)

well... that was longer than I intended! hopefully you find something to help! go, mama. you'll get through it! :)

@Cherrybee: Are you able to cancel any visitors for a couple of days so you can have a chance to rest and recover? I'm not an expert but I can't stress enough how important it is for you to be looking after yourself rather than worrying about the people who want to visit!

Eat plenty of good, calorific food to help your body recover from your traumatic delivery and to help get your supply going.

Drink plenty of water to help with any lingering oedema and again build supply.

Try to get outside into the fresh air everyday, enjoy the sun and get some vitamin d on your boobs!

Try and rest as much as you can - even if you can't nap during the day, take any opportunity to put your feet up and relax.

Hang in there Mama, you'll get there!

So this probably won't work with visitors around, but for me covering with breast milk and then letting them air dry worked best. So i was sitting literally with my shirt up for ten minutes after I finished pumping. I also just wanted to mention that it is normal to have low supply when the breasts are infected. I freaked out too about that when I had mastitis, but honestly once you are better your supply will increase especially since your baby is still really young- you have enough time to take care of that so one less thing to worry a about I hope. Just take care of yourself meanwhile. We are all rooting for you!

I've been terrible about keeping up with new babies, but CONGRATULATIONS on your sweet girl!

All purpose nipple ointment!! That may be the same thing @Mrs. Jacks was suggesting. Saved my nips. 9 weeks PP and things are still tender but oh so much better. We had to supplement for 4 weeks but have been 5 weeks on sole BM. Sounds like you're doing a great job keeping up supply, and more volume will come in time. You are a rock star <3

Oh honey! I went through a similar scenario!!! The first six weeks of Lala's life were filled with bloody infected nipples that caused her to vomit blood. I still have physical scars from the whole experience as well as mental scars from the lack of help that I received from the people who were there to help me.

Two things I found that helped:
1) soothies pads (which were like a religious experience), I kept them in the fridge and put them on for a half hour several times a day. Pure heaven!
2) special nipple cream from a compounding pharmacy. It contained nystatin, hydrocortisone and I don't know what the base was... But I found out the very hard way that I was allergic to lanolin based products. Can you say nipple fire?! It looked like I had poison oak on top of all the other hell going on with them.

And I emerged six weeks later tough as nails and ready to build my supply.

Hang in there cherry bee! It is hard!!!!

Oh good lord mama, I am so sorry! If you are on antibiotics make sure you take probiotics - you don't want thrush on top of all that :( it suckssss.

Wish I had more advice! I was told to rub bm on and let air dry as often as possible, only wash nipples w/water no soap. Hope you feel better soon!!

@Cherrybee: nipple shells!

http://www.amazon.com/Medela-SoftShells-Breast-Inverted-Nipples/dp/B001XDKTXQ/ref=sr_1_2?ie=UTF8&qid=1375470728&sr=8-2&keywords=nipple+shells

I've heard some people say that you have to be careful because thrush can develop (the last thing you need is more problems!), but I swear by these things. I wore them 24/7 for the first six weeks of my LO's life. I would slather on Lansinoh lanolin and then put these right in my bra over my nipple. I believe it gave my nipples a chance to heal!

Good luck!!!

FIrst of all, I am so sorry that you are having to go through this! I hope it all resolves quickly for you mama!

I always make sure to wash my hands really well before a feeding. After the feeding I would put some breaskmilk on them and let them air dry. Once they were dry I would do a nice coating of APNO ( all purpose nipple ointment - I had to get an Rx for mine) then top the whole thing off with a hydrogel.

So today I was diagnosed with mastitis in both breasts. The left breast also looks to have developed an abscess. I have actually pumped pus from the left one - so horrible.

A recap:

- Baby E has a tongue tie
- Mummy has large breasts and flat nipples
- The result - a terrible latch

Baby can't get much at all, never mind empty the breast so there was milk sitting in my breasts just waiting to get infected.

I have bought an electric pump to stimulate supply and I'm currently pumping and dumping. We are formula feeding in the interim.

In addition, my nipples are destroyed. Cracked, bleeding, open sores. Even pumping causes new wounds. We nursed using the shields for a day but even that caused nipple trauma. This is how the bacteria got into the breast to cause the infection.

So.... DD is having her tongue tie clipped next week and id like one last go at BFing (I doubt we will ever EBF as I only get 1oz every other pumping session but Id like to do a bit of both if I can).

So my question is, how do I sort my nipples out?? Go back to basics for me, how to you wash them and dry them after feeding? What nipple creams are good? Is there anything I can do to help them heal? I've tried putting breastmilk on them but I'm pretty sure they are rotting away!
