We did medium pretty quickly after she was born because slow was just too slow but it was a long time before we went to fast speed. Then shortly thereafter we started using straw cups. LO transitioned well from nipple to straw cup so we skipped the sippy cup stage altogether. I didn't notice any difference in how long it takes for him to finish a bottle. The fact that he still drinks from a bottle is another issue.
