There is an application form and you usually need to provide supporting medical documentation from your doctor. Our STD is connected to our sick leave, so your doctor can write you off for a specified period of time and it goes to our HR who submit it to insurance I think. The rules and policies will differ depending on which insurance company plan there is. </p> <p>There is EI sick leave too, but that would depend how far along you are.
