<description>&#60;p&#62;Yes, it cost $20 per person's FMLA claim (my short term disability was included too), so my husband and I had a total of $40.  It was a lot of paperwork, so I was happy not to do any of it.  &#60;/p&#62;
&#60;p&#62;The receptionist warned me about it when I dropped off the paperwork.  She was very relieved that I was ok with the cost, she said that people get real nasty with her about it, but she has no control over what the practice charges.

<description>&#60;p&#62;My doctors office charged me $5 to fill out my Temporary disability form. They didnt tell me ahead of time either but there is a big sign up on the wall that states there is a charge for this service but doesnt state how much. Personally I think its a bit ridiculous for them to charge for this but since it was only $5 I didnt complain.

<description>&#60;p&#62;Not that kind of paperwork, but my neurologist wanted me to pay $1 per page of my records (I wanted them to take to a new doctor) and there were over 40 pages. My mom called the office and pitched a fit. My dad and him are friends, plus she works in my dad's office (foot doctor) so when they have excuses for why they have to charge, she can tell them that she knows that's a lie. &#60;/p&#62;
&#60;p&#62;So we got them for free and now the copies are in a safe place so we won't ever have to deal with them again.

<description>&#60;p&#62;I don't know if my doctor charges $ for paperwork yet, but when I called our disability carrier Metlife to pre-register for short term disability they did warn me not to be shocked if the doc charges a fee to fill out the FMLA paperwork. So, I guess it must be sort of common? (still seems weird though, if you ask me).

Maybe they did charge me and I just didn't know...? I get that they should be paid for their time, but honestly how much time does it take to fill out and fax a form? And $25 is a LOT!

But in the past I've had to pay nominal fees for things like having copies of my medical records sent to school or something like that.  From their perspective it does take their time to do that for you, so they bill you for their time.  But I do agree it's a little crazy considering that you are already paying them a ton of money for your pre-natal care in general.

<description>&#60;p&#62;So today after my appointment, I went to pick up my FMLA paperwork that I need to turn in to my work and the receptionist told me it would be $25.00. I was confused and asked her to clarify and apparently they charge $25.00 to fill out paperwork! WTF? I was so annoyed. It's not a big deal or a lot of money, but I had never heard of such a thing. I mean, what would have happened if I refused to pay- they just wouldn't fill it out? I wouldn't be able to take leave? SO stupid.&#60;/p&#62;
&#60;p&#62;Has anyone else had to pay their doctor's office for paperwork?
